Position Summary


The Medication Coordinator is a Licensed Practical Nurse (LPN) who utilizes skills and knowledge to provide care for our clients as directed by our multidisciplinary team.

This role is a part of the integrated Health Services team.

The Medication Coordinator assists clients in accepting responsibility for their own health and assists them in achieving their health improvement goals through the delivery of prescribed medications and treatments.

Job Duties

  • Perform delegated independent nursing functions within the scope of practice as determined by CLPNA and the DI within the individual LPN competence level, as observed by the Nursing Coordinator
  • Maintains and ensures best practices around the handling of various medications, participates in the maintenance of medication administration and reconciliation procedures and documentation
  • Liaise frequently with pharmacy partners, both inhouse and communitybased, to ensure medications are delivered, sorted, stored, administered, wasted and/or returned to pharmacy are done so properly, according to regulatory body standards as well as organization protocol
  • Liaise frequently with physician partners, both inhouse and communitybased, to ensure a smooth client experience from medication prescription to administration, including all required associated tasks (ie. ECG or phlebotomy based on physician or pharmacist recommendations, communication with the pharmacy of choice, and receiving, storing, administration and documentation of medications, administered or missed)
  • Ensure compliance with regulatory standards (SDS, OH&S, WHMIS)
  • Organize job functions and work area to be able to effectively complete assignments within established time frames
  • Provide culturally aware support to DI clients, celebrating the diversity of clients and being aware of power dynamics of privilege and oppression that may be present
  • Ensure all required data is entered accurately into organizational databases including the DI Guestbook Database, and client Electronic Medical Records (EMR) and referrals provided in accordance to DI policy and professional nursing standards
  • Develop and maintain collaborative and accountable working relations with internal DI colleagues to strengthen communication flow and crossprogram understanding
  • Provide caring, reliable, traumainformed care to all patients & clients accessing the DI to reduce barriers to service and increase community trust
  • Conduct assessments, initiatives, and implementing health management strategies; provide appropriate evaluation, followup, and modifications to care plans according to practice protocols; assist with the delivery of disease management and health promotion; implement strategies for improving the quality and provision of care
  • Coordinate services with other team members and community resources according to policies, procedures, and established standards
  • Perform other related tasks that are approved by the designated Nursing Coordinator, Senior Manager, Health Services, and the Directors of The DI
Required Competencies

  • Current registration as an LPN with the CLPNA and proven experience as an LPN
  • Ability to work in a diverse care setting with a multidisciplinary team
  • Display compassion at all times, but commitment to standards and process
  • Completion of a university, college or other approved Practical Nursing program is required
  • Experience in acute care is an asset
  • Mental Health experience would be considered an asset
  • Current CPR certification is required
  • Energetic and selfdirected, with effective time management and organizational skills including the ability to deal with several priorities at the same time
  • Strong strategicallyfocused analytical skills, good common sense; and capacity to provide leadership
  • Welldeveloped interpersonal and conflict resolution and negotiating skills
  • Excellent communications skills, both oral and written
  • Computer skills including familiarity with word processing, database, and Internet
Working Conditions


Front-line staff are exposed to emotionally disturbing events, such as: medical distress, threatened/actual serious injury, violence, abuse, and death; where clients may be experiencing fear, terror, hopelessness, rage, self-harm and conflict.

It is understood and accepted that you will be working in distress and/or high-stress situations, which may include emergency response where resiliency and stress tolerance and the ability to maintain composure, remain calm and focus
