We are actively recruiting for a Patient Care Manager for the Palliative Care Unit and Allied Health Team


The Salvation Army Toronto Grace Health Centre (TGHC) is a mission of The Salvation Army that provides specialized and integrated care and services to individuals with multifaceted chronic or persistent illnesses that require Complex Continuing Care, Post Acute Care Rehabilitation, Palliative care and Transitional care services.

Position Summary:


The Patient Care Manager is an integral member of the inter-professional team and is accountable for leading, coordinating and supporting the delivery and provision of quality patient centred care.

The Patient Care Manager utilizes principles of management and best evidence and processes to achieve best patient outcomes.

The Patient Care Manager ensures that health care services are delivered to all patients and families, and actively contributes towards strategic directions and continuous improvement initiatives.

The principal responsibilities of the Patient Care Manager includes:

  • Ensures that individualized care plans for all patients requiring specific therapeutic programs are developed by the appropriate clinician/ therapist.
  • Facilitates the development of care standards which meet therapeutic patient requirements and that are compatible with standards for the relevant professions
  • Monitors staff adherence to documentation standards as per corporate policies and requirements from the relevant colleges
  • Evaluates Nursing and Allied Health workload measurement.
  • Directly supervises employees and participates in staff recruitment activities including interviewing, hiring and training employees; planning, assigning and directing work; evaluating performance; counseling employees as applicable.
  • Contributes to staff development by promoting and ensuring a supportive/collaborative learning environment and best practice standards for all staff
  • Collaborates with members of the multidisciplinary team to ensure high standards of quality and optimal management of client care outcomes (including data collection/reporting)
  • Supports ongoing continuous improvement practices to strengthen clinical processes and quality of care and patient safety.
  • Reports clinical/performance issues to the Director of Clinical Program and Transitional Units including potential solutions or options for resolution.
  • Ensures interprofessional approach to care recognizing the unique contribution of each discipline
  • Promotes collaborative practice, establishes processes and enablers to ensure effective teamwork and communication.
  • Develops and nurtures partnerships with internal and external communities
  • Participates as appropriate in hospital
- wide and external committees, projects and professional development activities.


Qualifications and Education:


  • Baccalaureate degree in one of the required Health Professions; Master's degree is preferred
  • Member of Regulated Health Profession in good standing
  • Certification in Palliative Care an asset
  • Basic Cardiac life Support (B.C.L.S.) program required.
  • Demonstrates membership within a professional association and/or professional interest groups related to practice preferred
  • Minimum Three (3) years of relevant management and clinical experience, including experience working in palliative care and complex continuing care patients, frail Seniors, and patients with multiple comorbidities including persons living with various degrees of cognitive loss or history of mental health illness / needs.
  • Demonstrates exceptional interpersonal and communication skills (verbal and written), including conflict resolution, negotiation, ability to effectively facilitate difficult conversations.
  • Ability to model leadership behaviors and create a positive work environment where staff are motivated to do their best and strive for continuous improvement
  • Maintains and protects patient confidentiality in accordance with relevant legislations and organizational patient privacy and confidentiality policy requirements
  • Client service oriented, with the ability to effectively work within a framework that supports diversity, inclusiveness and equity.
