    Job summary

    Newcastle Gynaecology is looking to recruit a Cancer Care Coordinator to work offer patient and pathway support, along side our Cancer Nurse Specialists. The role will integrate into a dynamic team committed to service improvement and excellent patient experience.

    This exciting role would be suitable for an experienced Administrator. You will use your experience and knowledge to support and complement our existing team providing and monitoring patient-centred care across a number of Gynaecological cancer pathways serving patients throughout the North East of England

    The post holder will be based within the Gynaecology department at the the RVI in Newcastle. You will be caring, motivated, proactive, and committed with excellent communication skills. You will work independently and as part of a team under the leadership of the Admin Manager with close support and mentorship from our Cancer Nurse Specialists. You will support with all aspects of improving the safety and quality of patient care at every opportunity.

    The role will involve working within core hours Flexibility is required to support patient and service need.

  • Main duties of the job

    The role of the Cancer Care Coordinator is to support the Gynaecological Cancer Care Pathways. The team is comprised of this position, Gynaecology Administrators and Cancer Nurse Specialists.

    It is a busy department managing referrals from primary care in the Newcastle area and tertiary referrals from neighbouring Trusts. The Cancer Care Coordinator role is predominantly focused on supporting patients through the post-diagnosis phase of their treatment pathway and ensuring timely treatment by monitoring waiting times, facilitating investigations and treatments and following up on results.

    This is a proactive role and the post holder will be involved in continuous service improvement by collecting, organising and presenting data and working with management and nursing colleagues to co-create improved systems and processes.

    The role involves regular patient and family contact to support the care and emotional wellbeing of those affected by cancer. The role involves working with partner departments and teams within and outside of Newcastle upon Tyne Hospitals Trust, advocating for patients where appropriate, to provide an efficient and seamless patient experience.

    Job responsibilities

  • Triage incoming calls, using a risk assessment framework and initiate appropriate response according to protocols and individual pathways, using good communication skills, basic clinical awareness and appropriate tools and procedures, liaising as appropriate when non routine and refer complex decisions to the team for assessment and review.
  • Document and monitor all aspects of care coordination and service delivery, supporting data collection for audit
  • Support tracking of patients on the cancer pathways to support achievement of national cancer targets.
  • Coordinate the care for a defined group of patients assessed by a registered practitioner.
  • Organise and prioritise the designated workload in relation to identified needs
  • Contribute to the holistic needs assessment and the development of an individual care plan
  • Coordinate the handover with other teams to facilitate safe and effective transition of care between services in order to provide seamless support for people
  • Deliver patient-centred, self-management support and education as necessary to non-complex patients, including how to self-assess
  • Coach patients, carers and appropriate staff to understand what signs, symptoms or situations to be aware of that would indicate concern
  • Coach patients and carers on how to make contact when they feel that their condition or needs have changed, including what to do out of hours
  • Attend and support the local weekly MDT and patient tracking meeting and activity.
