Home and Community Care Support Services Champlain is one of 14 Home and Community Care Support Services organizations in Ontario.

We are dedicated to ensuring our more than 64,000 patients annually receive the highest-quality health care and services at home.

Our patient-care coordination teams help develop care plans for people of most ages, focused on maintaining each patient's independence and dignity.

We also provide long-term care home placement services, and facilitate our patients' access to the community-based services they need.


Position Summary:

Reporting to the Director, Home and Community Care, this position will focus on transformational planning/implementation and projects to help guide the home and community care department towards its mandate


Primary Responsibilities:

Delivery of Services

  • Applies sound management theory and concepts to leading, staffing, monitoring, evaluating, and data gathering including the development of relevant outcome measures pertinent to his/her respective line of business area
  • Promotes and manages leading client services delivery models and implements best practice
  • Facilitates and role models effective negotiation and problem resolution
  • Anticipates, recognizes and meets internal/external clients' needs
  • Provides point of care resources to ensure safe and exceptional client care
  • Identifies and implements opportunities for service improvement and enhancement
  • Contributes to the development and is responsible for the implementation of a line of business vision
  • Reviews and simplifies systems/structures/policies to support the vision of the line of business and HCCSS Champlain
  • Identifies potential market and strategic opportunities for the program in the community
Relationship Management

  • Identifies opportunities for collaborative partnering with external providers/organizations that support strategic direction
  • Maintains/promotes effective working relationships and influences external stakeholders to achieve organizational goals, integrated services and quality client care
  • Leads and empowers crossfunctional teams drawn from across Champlain and Ontario in joint problem solving and cocreation with respect to developing and delivering improvement initiatives with emphasis on programs within portfolio
  • Maintains and promotes effective and collaborative working relationships with employees at all levels of the organization to achieve organizational goals and fosters a client driven, continuous learning culture
  • Creates a shared vision and synergy in teamwork. Works with others toward shared goals
  • Demonstrates ability to guide individuals and groups while maintaining group cohesion, motivation, commitment, and effectiveness
Quality Improvement/Risk Management

  • Actively participates in developing and implementing evaluation and quality improvement programs with a focus on outcomebased care and evidencebased practice.
  • Monitors and reviews risk with regards to complex care situations and ensures appropriate documentation and accountability for resolution
  • Researches and analyzes needs/trends and program outcomes to anticipate and predict the need for change
  • Demonstrates ability to effectively introduce and manage change that is consistent with the strategic plain, vision, purpose, and operating principles of the HCCSS
  • Acts as a champion for continuous quality improvement and participate in developing policies, procedures, systems and tools to improve service delivery

Education and Experience:

Education and Experience

  • Completion of an Undergraduate Degree in a health related discipline
  • Formal management education and management skills development
  • Minimum of three (3) years related management experience, preferably in a community health setting
  • Membership in good standing in a professional regulatory college
  • Experience leading in diverse and unionized community health care environments

Knowledge:


  • Knowledge of standards of practice relevant to professional associations, (Nursing, PT, SW, OT, RT, RD, SLP.)
  • Knowledge of direct care/case management/care coordination models used in community health care organizations and roles of health care professionals
  • Knowledge of community needs, partners and resources
  • Knowledge of general human resources management practices including working in a unionized environment
  • Knowledge of budget planning and financial accountability and reporting

Skills and Abilities:


  • Ability to deliver information effectively, verbally and in writing, in a variety of settings including oneonone, team meetings, and presentations
  • Presentation skills to convey information, decisions and recommendations to managers, senior leaders and community groups
  • Effective negotiation and conflict resolution skills to resolves issues with staff, unions, external partners and contracted providers
  • Facilitation skills to work with employees, LHIN leaders, and community partners in develo
