Connect with Fraser Health on social media for insights from employees. **Detailed Overview** In line with Fraser Health's Vision, Mission, and Values, this role supports individuals and families affected by mental illness and substance use disorders. The focus is on advocacy, counseling, education, community support, and care coordination. The aim is to ensure consistent, relevant, and timely treatment and access to information when needed. Services may include outreach to clients in their environment. **Responsibilities** - Develop and implement treatment plans in collaboration with clients - Provide support through individual, group, and family sessions - Assess client needs and determine best approach to care - Facilitate client access to community services and programs - Advocate for clients and provide necessary support - Maintain accurate client records - Provide follow-up care and referrals - Participate in quality improvement activities - Stay updated with relevant practices and legislation - Take part in research and program evaluation - Perform other assigned duties **Qualifications** **Education and Experience** - Bachelor's degree in Social Work, Clinical Counselling, or related discipline - Two years of relevant experience working with clients with mental illness and substance use disorders - Eligible for registration or membership with professional association - Valid Class V BC Driver's License and access to a vehicle **Skills and Abilities** - Client assessment and case management skills - Effective communication verbally and in writing - Interpersonal skills to work with clients, families, and team members - Ability to prioritize, organize work, and exercise sound judgment - Work independently with minimal supervision - Maintain confidentiality - Adapt to changing needs and manage pressure - Learn new concepts quickly - Knowledge of community resources - Understanding of emergency triage procedures and counseling principles - Physical ability to perform job duties
