    Description
    Position Summary

    Under indirect supervision, and working as part of an interdisciplinary team, the Registered Dietitian is responsible for providing high quality professional nutritional care and counseling to assigned patients, consistent with the philosophy of Orillia Soldiers' Memorial Hospital.

    The Registered Dietitian is responsible for providing comprehensive nutritional therapy to patients and their families through education, guidance, and counseling. In addition, the Dietitian is required to contribute to program planning, implementation and evaluation in a multidisciplinary team concept.

    The Bariatric Program (CHAT) focuses on strategies to empower families to eat well, be active and feel good. The Diabetes Program focuses on strategies to coach and mentor families to manage glycemic control through medication, healthy eating, activity and stress management. Together both of these programs support a strength based approach to nutrition.

    The Paediatrics Diabetes and Bariatric Centre will work with a targeted population of First Nations individuals up to an including age eighteen (18).

    PRIMARY RESPONSIBILITIES:

    Manages Nutrition Care:

    •Assessing individual needs and extent of nutrition care required.

    •Planning, implementing, monitoring and evaluating nutrition care provided.

    •Promoting self-care by encouraging a sense of responsibility and promoting a positive attitude towards the acceptance of diabetes.

    •Collaborating with other team members and/or outside agencies contributing to a total client care approach to diabetes management.

    •Educating and counseling clients/families individually and in groups to assess relevant information and formulate a nutrition-care plan.

    •Establishing goal oriented treatment interventions.

    •Documenting care and treatment plans in the health record and to the referring physician/agency as necessary

    •Using Family Based education principles throughout the education process.

    Manages Resources, Systems and Change:

    •Familiarizing self with philosophy, policies, standards, goals, and objectives of the Hospital and the PDEC.

    •Participating in the development of the PDEC's mission and short and long term goals and objectives.

    •Participating in the development and implementation of the PDEC's policies and procedures.

    •Striving to improve the program's efficiency and effectiveness through creative thought and innovation.

    •Using reliable and up to date nutrition education resources, and if need be, developing and distributing nutrition specific educational materials.

    •Carrying out a needs assessment, developing, implementing, and evaluating the teaching plans and educational material.

    •Providing direction and instruction to students and evaluating their performance as required.

    •Participating in workload measurement.

    •Providing input into recruitment of dietitians.

    Communication:

    •Collaborating with other health care agencies/professionals regarding current diabetes management practices.

    •Providing nutritional consultative support to health professionals, dietetic interns, community groups, government, media and individuals

    •Maintaining strict confidence concerning clients' medical and personal information.

    •Consulting with the referring physician and/or the PDEC Medical Director on matters relating directly to the medical management of diabetes and direct client care issues.

    •Providing workshops and in-services to other health professionals, other institutions, and the community

    •Acting as a consultant to health professionals and the public on diabetes management issues.

    •Attending and participating in departmental and Ontario Bariatric Network meetings (OPBN) and Provincial Council for Maternal and Child Health (PCMCH) meetings as required.

    •Increasing public awareness in relation to nutrition/healthy living through public displays, seminars and liaison with other service providers.

    •Acting as a resource within the FHT on nutrition related issues.

    Professional Self Development:

    •Maintaining Diabetes Educator Certification and license with the College of Dietitians of Ontario. Consider membership with Dietitians of Canada.

    •Maintaining Bariatric Educator Certification under Obesity Canada.

    •Attending appropriate workshops, professional conferences, in-services and participate in e-learning, journal and literature reviews as required to update knowledge and skills pertaining to diabetes/bariatric education and management.

    •Interpreting, evaluating and applying current research.

    •Developing and documenting continuing education plans and activities.

    •Participating on committees and in clinical projects/studies as required.

    •Liaising with other health professionals.

    Performs other duties as may be assigned by the Program Manager and Director.

    Qualifications

    Education:

    •Completion of a Bachelors Degree in Nutrition, Dietetics, or related field and a graduate of an Accredited Dietetic Internship program required.

    •Current member of the Ontario College of Dietitians required.

    •Certified Diabetes Educator (CDE) status preferred/achieved within 1 year of employment

    •Certified Bariatric Educator (CBE) status preferred/achieved within 1 year of employment

    •Membership in the Dietitians of Canada considered an asset.

    Experience:

    •Experience with Diabetes Education preferred.

    •Minimum of two (2) years' experience working within a Paediatrics setting.

    •Experience with electronic medical records system is an asset.

    •Experience with group education, presentation and public speaking.

    Competencies:

    •Demonstrated knowledge of chronic disease prevention and management.

    •Knowledge of insulin therapy, to include MDI, CSII, and continuous glucose monitoring systems. Will be required to obtain and maintain insulin adjustment competency and medication dispensing certification requirements.

    •Demonstrated computer literacy with some advanced computer skills.

    •Demonstrated strong communication skills in English, both written and verbal.

    •Demonstrated ability to organize, prioritize and multi task efficiently.

    •Demonstrated ability to maintain confidentiality and work in a highly professional manner.

    •Demonstrated ability to educate, facilitate and present information clearly and precisely.

    •Demonstrated ability to work both independently and in a team environment.

    •Demonstrated ability to deliver individual and group counseling in French is an asset.

    Other:

    •Must have a valid driver's license and a reliable vehicle to provide client care / services off-site
