Home and Community Care Support Services Champlain is one of 14 Home and Community Care Support Services organizations in Ontario. We are dedicated to ensuring our more than 64,000 patients annually receive the highest-quality health care and services at home. Our patient-care coordination teams help develop care plans for people of most ages, focused on maintaining each patient's independence and dignity. We also provide long-term care home placement services, and facilitate our patients' access to the community-based services they need.


Position Summary:


Reporting to the Director, Patient Care Contract Management, the Manager, Patient Care Contracts position is responsible for negotiating, managing, monitoring and evaluating contracted service provider performance according to established Government/ministry directives, service provider/vendor contractual agreements and Home and Community Care Support Services (HCCSS) goals and objectives.

Responsible for participation in establishing and managing the contract management framework for contracted service providers and vendors.

Provides leadership/mentorship for patient services, quality risk colleagues, and service providers on contractual, service related and/or client issues to de-escalate and mitigate risk.

Accountable for implementation and sustainability of new initiatives and programs including revising and developing supporting policies and practices. Provides expert advice to internal and external stakeholders on performance, accountability and patient care procurement matters as required.

Demonstrates excellent skills and abilities to develop effective working relationships, provide leadership and build effective teams with both internal and external stakeholders.

Also provides project management, direction and leadership to all HCCSS staff as appropriate and direct supervision/mentorship of designated members of the team.


Primary Responsibilities:


  • Supports the Director, Patient Care Contract Management in instituting local and provincial administrative policies and practices for the ongoing management and oversight of existing contracts, including but not limited to, volume and market share management, contract performance management, and initiation of corrective contractual remedies when necessary to ensure safe, quality care for patients
  • Contributes to analysis of local and regional system performance within the context of provincial performance goals, and assesses the implications for management of contracted service provider and vendor performance targets
  • Reviews and validates performance reports, determines performance against contract specifications and makes recommendations to address emerging issues or requirements for remedial action in consultation with the Director
  • Maintains oversight of contract CHRIS database to meet contractual obligations
  • Leads and/or collaborates with internal and external stakeholders in quality improvement initiatives
  • Provides leadership to staff reporting to them as required, and monitors their work, and priorities during projects and assignments; this includes ensuring adherence to maintaining all appropriate and required documentation and record keeping
  • Other duties as assigned, including supporting various committees and working groups in achieving the goals of HCCSS and the ministry
  • Performs oncall support to operational divisions after hours if required

Education and Experience:

Educational Qualifications

  • A degree, in business administration, health sciences, or health discipline
  • Evidence of continuous learning related to health and/or contract law/ project management
  • Appropriate accreditation/designation (Ex. CSCMP, RN, BN, PMP).
  • Master's Degree an asset
Experience
years' experience in a health care or relevant environment

  • Experience leading and participating in negotiation of agreements, conflict resolution or arbitration
  • Proven ability to effectively manage confidential, complex and sensitive information, agreements and contracts
  • Experience in Contract Law, Broader Public Sector, Ontario Public Sector, and Patient Service Procurement Directives
  • Experience working with SPO and Vendor contracts, selfdirected care programs, private/home school, hospice, Transitional Care, and retirement home funding processes
  • Experience performing financial analyses and critical evaluations of complex financial programs
  • Experience performing evaluations, interpreting and analyzi
