- The role will include:
- In collaboration with the patient/family/caregiver and other health care professionals, help formulate a plan of care based on patient assessment.
- Evaluate the effectiveness of treatment plan(s) and adjust accordingly in consultation with the patient/family/caregiver and other health care professionals.
- Act as a patient advocate by protecting and promoting patient rights.
- Travel within your region between patients' homes
- Providing services in a variety of settings which may include in-home, clinic, retirement home, or school
- Performing other clinical duties as required
- Experience in wound care, IV medications and re-sites, catheter care, compression/VAC, and ostomy care considered an asset
- Delegate as required within your scope of practice
- Diploma from a recognized RPN program or equivalent
- Registered Practical Nurse in good standing with the College of Nurses of Ontario
- Experience as a visiting nurse, visiting registered nurse, visiting registered practical nurse, and/or community nurse is considered an asset
- Physical ability to stand, bend, walk, crouch, kneel, and reach, ability to lift 50lbs (23 kg) independently, ability to assist clients with standing, walking, and sitting; ability to reposition clients in bed
- Valid Standard First Aid and Level C CPR
- Updated immunization records (including completion of a two-step tuberculosis test within the last 6 months)
- Clear criminal background checks and Vulnerable sector checks are required upon hire
- Ability to interact with others in a caring, tactful, and courteous manner
- Excellent problem-solving, time management, and crisis-resolution skills
- Excellent interpersonal and communication skills, both verbal and written
- Valid G2/G driver's license and access to a reliable car. This position requires daily travel of more than 30 minutes, so 100% must have a vehicle. Candidates without a vehicle will not be considered.
