We are looking for a Coordinator/Case Manager to join our Empowered for Employment - Peering into Employment team Peering into Employment is a coordinated housing and employment program for individuals who are at risk of or experiencing homelessness.

Empowered for Employment Services offer various programs that provide comprehensive support to diverse clients including survivors of violence and/or abuse, mature workers 55+, persons with disabilities, and people at risk of or experiencing homelessness.

Position

Reporting to the Programs Manager, Empowered for Employment, the Coordinator/Case Manager is responsible for delivery of our Peering into Employment program, a coordinated housing and employment program for individuals who are at risk of or experiencing homelessness.

The program's intent is to explore peer employment's efficacy on participant well-being including mental health, physical health and movement along the labour market continuum.

The Coordinator oversees the Peering into Employment program operations in conjunction with case management of individuals facing complex challenges, providing advocacy and coaching to enhance personal management and employability.

The Coordinator assists with housing, personal supports, employment planning and work exploration, skills training, job search, and coordinates work experience through meaningful peer employment opportunities.

The Coordinator empowers individuals to engage and connect with their communities and establishes collaborative relationships with community and service providers to promote program and continuity of required supports for program participants.


General Responsibilities:
Oversee day-to-day operations of program with emphasis on meeting service delivery standards and targets per project/ funder agreement.
Supervision, work direction, and guidance to program staff in accordance to the needs of program and participants.

Undertake regular community engagement and outreach activities to develop and maintain strong community connections to promote program, build referrals and foster business/employer partnerships to facilitate peer employment and traditional employment opportunities for program participants.

Conduct participant intakes including housing and employment assessments to determine housing, employment, and other personal needs.

Develop service plans identifying participant goals, strengths, constraints, needs and activities to work toward identified goals and objectives and build job readiness.

Assist participants to access income supports, secure housing and determine lived experience and peer employment goals.

Facilitate group and/or one on one employability skills training: life skills, health & wellness, Skils for Success, career/job exploration and job search strategies.

Coordinate quarterly progam advisory meetings in collaboration with program participants.

Coordinate external training opportunities and manage and administer required financial supports; housing, skills training/courses, transportation, food, clothing, job start supplies, etc.

Ensure timely services and provide necessary coaching and support and skills development to enhance employment readiness.
Accompany participants to meetings, housing viewings, placement interviews, peer employment and other stabilization advancement appointments as necessary.

Provide on the job coaching as necessary and maintain regular follow up during peer employment to ensure a successful peer experience and placement.

Maintain thorough case files and timely documentation in Sharevision including all participant communication, activities, and outcomes.
Maintain regular tracking of referrals, services provided, outcomes, and other statistical data as required by SOURCES, manager, and funder.
