Our growing organization has multiple Care Coordinator vacancies at clinics in Ontario and British Columbia locations.

Ontario applicants will only be required to work out of our Ontario clinics and British Columbia applicants are only required to work out of our British Columbia clinics.


ROLE SUMMARY

RESPONSIBILITIES:


  • Coordinates a caseload of new and ongoing concussion and pain patients through their journey to recovery.
  • Provides proactive communication to patients and various stakeholders (legal counsel, insurers, employers, insurance provider, family physician) involved in the patient's recovery apprised of rehabilitation and claim status.
  • Partners with internal practitioners to advocate on behalf of the patient and to coordinate rehabilitation.
  • Explains the treatments, milestones and expected outcomes of prescribed rehabilitation.
  • Supports the creation and submission of treatment plans that are catered to the patient's unique needs in partnership with the interdisciplinary treatment providers.
  • Identify and address any issues or barriers that may impede patient's progress.
  • Additional duties and responsibilities as needed.

QUALIFICATIONS:


  • One (1) year experience in a rehabilitative care environment
  • One (1) year experience working with accident benefit claims (Ontario).
  • Two (2) years customer service experience
  • Minimum Grade 12 Secondary School Completion.

KNOWLEDGE, SKILLS, AND ABILITIES:


  • Medical terminology
  • Knowledge of WSIB and MVA claims (Ontario)
  • Knowledge of ICBC and WSBC (British Columbia)
  • English proficiency and communication
  • Ability to communicate with legal counsel.
  • Ability to work in a fastpaced environment under supervision as well as independently.

  • Organization and time management skills
  • Strong documentation skills
