Primacare Living Solutions is currently looking to fill a role of a social worker for our Long-Term Care Home in Brampton - Burton Manor.


In this role the incumbent will provide social work services 3- 4 days a week to our residents and their families.


Responsibilities:


  • Providing Support and Counselling to Residents and Families
  • Facilitating admissions, discharges and transfers
  • Facilitating tours of the home for new residents and families
  • Attending and participating in Care Conferences
  • Work with all members of the interdisciplinary team

Qualifications

  • Master's degree in social work
  • Membership in the Ontario College of Social Workers
  • Experience working in a similar or same role in Long Term Care
  • Knowledge of applicable regulations

  • 8 hour shift

  • Master's Degree (preferred)

Experience:

Long Term Care: 2 years (preferred)


Licence/Certification:

  • Ontario College of Social Workers (preferred)
