- Assesses, plans, implements and evaluates nursing care of assigned patients byassessing actual or potential problems and strengths, plans and performs interventions and evaluates outcomes.
- Collects information about the patient usinga variety of sources such asskills of observation, communication and physical assessment to enable appropriate nursing care.
- Communicates and consults with members of the health care team about the patient care plan; participates in interdisciplinary conference andexplains plan of care to the patient and/or families.
- Acts as an advocate to protect and promote the patient's rights to autonomy, respect, privacy, dignity and access to information.
- Documents timely and accurate reports of relevant observations including patient and family teaching and evaluation of nursing care.
- Participateson nursing, hospital committees and councils for improvement of patient care and/or work environment.
- Participates in the orientation and ongoing education of nursing staff and students by providing information and acting as a preceptor, where appropriate; identifies self-learning needs and attends educational programs to maintain and enhance clinical competency.
- Participates in quality improvement activities by identifying patient care issues and collecting data,evaluating patient care programs, techniques, standards and procedures by providing feedback about effectiveness and patient care outcomes.
- Maintains a safe environment for patients, families and staff by adhering to established safety and emergency practices and procedures.
- Performs other related duties as assigned.
- Knowledge of nursing practice as it pertains to hospice palliative/residencepatients
- Knowledge of other healthcare professions and their contribution to the multidisciplinary team
- Ability to work independently and as a member of a multidisciplinary team
- Ability to communicate effectively both verbally and in writing
- Ability to organize and prioritize work in a dynamic environment
- Ability to develop and maintain rapport with the patient, family and caregivers
- Ability to operate related equipment including applicable software applications

Detailed OverviewAs a member of a multidisciplinary team, provides direct nursing care to patients of the Hospice Residence at assigned locations according to established policies and procedures by assessing physical, emotional, spiritual and psychosocial needs; plans, documents and implements care plans; evaluates the effectiveness of the care plan and makes recommendations for care plan changes,where necessary.
ResponsibilitiesGraduation of an approved School of Nursing and/or Psychiatric Nursing. Two (2) years' recent, related medical nursing experience in an acute care facility, including one (1) year recent related experience in a hospice/palliative care environment, or an equivalent combination of education, training and experience.
Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
