Description
Collaborates with the resident, family/caregiver, and health care team in the development, implementation and evaluation of the residents' plan of care.
Provides leadership and mentorship to staff that enhances their knowledge, assessment skills, and ability to care for residents. Leads and collaborates in research, education, and evidence-based practice initiatives to optimize the resident, LTCH and health system outcomes.In collaboration with the Medical Director and Attending Physicians, provides comprehensive primary care to residents.
# Collaborates and consults with physicians, the Director of Care, nursing staff, interdisciplinary team members and external resources regarding residents' plan of care.
# Provide direction and leadership to RN's, RPN's, PSW's, and other direct care staff in all aspects of resident care, within established legislative standards.
# Makes referrals to specialized consultants, services, and other health providers.
# Complete nursing documentation, and assist residents with move-in.
# Is inspirational and passionate about quality dementia care and capturing positive moments.
# Ensure medication reconciliation occurs as needed.
# Identifies and implements research-based innovations for improving resident care and collaborates with members of the interdisciplinary team and/or community to identify research opportunities and to conduct and/or support research.
# Implement resident-focused programs, policies, and procedures with the Director of Care and the Quality Team.
# Registered Nurse Practitioner in good standing with the College of Nurses of Ontario, or a Registered Nurse Practitioner registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60.
Post-graduate education and experience in gerontology (recommended).
Specialty certification in gerontological nursing (recommended).
Comprehensive knowledge of long-term care concepts and philosophies.
Experience in long-term care, with providing empathetic care for the elderly.
Experience providing leadership and supervision in any healthcare environment.
Ability to embrace various skills from the team to maximize the potential to enhance the well being of all individuals.
Must have a clear vulnerable sector screen, no older than 6 months, TB test results less than 6 months old, must be able to provide three supervisory references.
