Opportunity
qathet Complex Care Housing is a collaboration between LIFT, Vancouver Coastal Health (VCH) and Tla'amin Nation.

This program represents a suite of services and supports aimed at ensuring people with significant mental health, substance use and other complexities attain safe and stable housing as a foundation to thrive.

These services support participants through transitions with the appropriate level of service as their needs change over time, in line with Tier 1, Intensive Supportive Housing and Tier 2, Stabilization and Respite, and represent an increase in services offered at Supportive Housing.


LIFT is seeking a
Clinical Coordinator to join the qathet CCH team and help provide clinical supervision and operational support to the CCH clinical team and the program's clients.

Summary
The Clinical Coordinator is a member of the Complex Care Housing team.

Reporting to the Lift Director of Programs, and in collaboration with the Complex Care Housing team, the Coordinator provides front line leadership for their team while providing both direct nursing care to participants and administrative support to the program.

Responsibilities

Clinical Practice and Client Care

  • Coordinates patient/client care activities to deliver primary care, mental health and substance use services
  • Facilitates efficient client flow and access within and between units and services
  • Collaborates and consults with the interdisciplinary team, other health care professionals/partners, and clients/families to establish appropriate goals and holistic care plans.
  • Assist with therapeutic interventions, problem solving, and complex cases as needed.

Administrative Duties

  • Assists the Manager with performing administrative duties and coordinates staffing and scheduling ensuring optimal use of patient/client care resources.
  • Provides input into proposals, policies and procedures
  • Promotes patient/client care in the unit/program by evaluating clinical practices and systems and drafting new and/or revised procedures, standards and criteria in conjunction with the interdisciplinary team and patient/clients/families
  • Supervises and guides designated staff
  • Provides leadership in the planning and implementation of innovative approaches for delivering quality and efficient services to support/promote health and illness prevention strategies.
  • Acts as a clinical resource for the multidisciplinary team (and students). Provides guidance/knowledge in the management of client care in defined area of clinical expertise.
  • Provides input into employee performance evaluations, discusses and clarifies performance expectations with staff, mentors/coaches team members to meet goals and performance targets, identifies and suggests staff training/skills requirements and enhancements, documents performance problems, and provides input to Manager regarding probationary period extensions and disciplinary action when required.
  • Coordinates staffing/scheduling activities. Plans, schedules, modifies, and approves work schedules, rotations, vacations, leave, and sick relief ensuring daily schedules are prepared. Maintains timekeeping records and communicates with payroll on issues such as changes in timekeeping procedures, employee inquiries, and corrections to resolve any discrepancies.

Education and Experience

  • Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM) is required.
  • Completion of post basic specialty nursing certificate/program if applicable
  • Three (3) years' recent, related experience in the designated clinical area including one (1) year's related administrative/supervisory experience or an equivalent combination of education, training and experience.
