    Detailed Overview

    Responsible for overseeing and coordinating the local delivery of Primary Health Care services by administrating, planning, developing, implementing, and evaluating the effectiveness, efficiency and accessibility of the services. Monitors, reviews, and revises policies, procedures and standards, supervises designated staff, orientates, trains and evaluates staff performance and plans for ongoing staff training needs.

    In addition, provides education about illness prevention, health maintenance and health promotion strategies through individual counseling, instruction, facilitating groups and conducting formal presentations. Utilizes a community engagement framework to engage and motivate a broad range of internal and external stakeholders to further a population health approach designed to enhance community health and well-being and, ultimately, reduce the burden of chronic disease.

    Collects and analyzes statistics utilizing caseload statistical data and community needs, participates in the budget process, recruitment and selection of staff and consults and liaises with other Primary Health Care staff, community groups, and agencies.

    Responsibilities
    1. Oversees and coordinates the delivery of Primary Health Care services by planning, developing, implementing and evaluating the effectiveness of the local delivery of the services provided to the Community. Monitors, reviews and revises policies, procedures and standards related to the care provided and collaborates with the Manager, Clinical Operations as required.
    2. Recruits, interviews and selects staff; provides leadership by developing, implementing and evaluating discipline specific staff orientations, in service education and placement of students for clinical practice experience.
    3. Supervises designated staff by methods such as scheduling and coordinating work assignments and delegating work activities in accordance with the service needs. Evaluates and corrects employee performance, conducts performance evaluations, ensures staff meet and maintain professional nursing standards of practice through a process of education, observation and evaluation and reviews nursing documentation for accuracy. Collaborates with the Manager, Clinical Operations and others to identify and recommend training/skill enhancement for designated staff.
    4. Plans, implements and evaluates orientation of new Primary Health Care Nurses and designated staff; arranges student nurse field experience as required in collaboration with educational facilities.
    5. Ensures staff are familiar with program policies and directives through a process of orientation and by regular communication and updates and identifies local Primary Health Care goals and objectives.
    6. Implements and monitors operating budget, provides input into the purchase of equipment and orders equipment and supplies, as needed coordinates the trial and evaluation of new equipment and supplies.
    7. Establishes and promotes positive relationships within communities to foster positive culture change around the critical role of social determinants of health in influencing individual and collective health and well-being. Liaises with Aboriginal Health, community agencies, schools, and various health related committees and communities around healthy living strategies.
    8. Performs nursing assessments, care planning, intervention and referral services for patients/clients of all ages with a variety of acute and chronic illnesses/diseases including communicable diseases, addictions and general health issues. Performs nursing procedures as delegated by primary care physicians.
    9. Provides health counseling and monitoring, as required, using a health promotion/prevention approach with individuals and groups. Instructs patients/clients/families/community members about illness prevention, health maintenance and health promotion strategies.
    10. Develops and conducts presentations for service providers, students and a variety of health resources designed to educate, facilitate and enable clients to direct their care and make decisions appropriate to their well-being.
    11. Develops and maintains health education resources and program materials; identifies clinical research projects and participates in the development, implementation an evaluation of educational components.
    12. Participates in the development of standards of nursing care, and long term planning by reviewing, evaluating, and making revisions and recommendations to the Manager, Clinical Operations.
    13. Contributes to the development and implementation of new programs to meet emerging local health needs and/or identified priorities.
    14. Participates and provides a leadership role in quality improvement and risk management activities by evaluating nursing practice, generating recommendations for alternative approaches, conducting safety audits and taking corrective actions; consults with Manager prior to changes in current practice.
    15. Promotes evidence-based practice through methods such as collaborating with others and promoting staff awareness and involvement in research and educational activities, identifying practices/issues that require research and collaboration in the development of ongoing education.
    16. Participates in local and regional health planning by attending meetings and committees, facilitating the resolution of issues and representing Primary Health Care services and community interests.
    17. Collects and analyzes statistics and identifies trends and issues in Primary Health Care.
    18. Follows-up on incidents concerning staff and/or clients by methods such as investigation, collecting data, conducting safety audits and taking corrective action, compiling incident follow-up forms and ensuring that critical incident stress debriefing sessions are held as appropriate.
    19. Participates on assigned committees as a representative of the Primary Care.
    20. Performs other related duties as assigned.
    Qualifications
    Education and Experience
    Bachelor's Degree in Nursing from an approved school of Nursing. Five years' recent, related clinical experience working in a community setting, including one year supervisory experience of multi-disciplinary staff. Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Valid BC Driver's License and access to personal vehicle for business-related purposes.
