Primary Responsibilities:


  • In collaboration with the team and manager, organize and prioritize daytoday patient care activities, staffing, scheduling and resource utilization
  • Round, monitor and address patient and family satisfaction with care
  • In collaboration with the manager, facilitate individual and interprofessional team rounds, and effective work processes
  • Facilitate staff orientation and identify staff learning needs and appropriate resources in collaboration with program Educators & Manager
  • Support the accountability of team members to the Manager with regards to patient care, professional conduct and performance
  • Participate in hiring, orientation and performance evaluation of staff
  • In collaboration with manager & program leadership, support THP & program strategic plan
  • Liaise with members of interprofessional team (inpatient and outpatient) to ensure excellence in patient care
  • Regularly completes chart reviews and audits and followups to ensure adherence and compliance to standards of care
  • Plan, coordinate and case conferences and as required
  • Monitor and address health and safety inspections
  • Monitors the quality of patient care through daily huddles and monitoring of key quality metrics on the Quality Board
  • Assists in the development of effective interpersonal relationships among interprofessional team through coaching, mentoring and facilitating difficult conversations

In addition to the above, this position includes a reduced clinical caseload focussed on family education_**
Key Qualifications:
  • Masters of Social Work (MSW) / Masters of Occupational Therapy (MScOT)
  • Minimum five years current Mental Health experience, primarily with youth and adults (from 1435 years of age) experiencing psychosocial issues, psychiatric conditions or mental health disorders
  • Experience in developing and implementing clinical programs
  • Knowledge or experience related to psychosis
  • Experience with case coordination, counseling and group facilitation and family education
  • Expertise in evidence based treatment modalities such as Cognitive Behavioral Therapy for Psychosis or Action Based Cognitive Remediation Therapy.
  • Effective interpersonal and organizational skills
  • Ability to work independently as well as part of a multidisciplinary team
  • Knowledge of the NAGIVATE Model preferred
  • Demonstrated commitment to patientcentered care
  • Role models excellence in patient care and serves as a mentor for the team, facilitating growth and development of team members with initiative and creativity
  • Demonstrated excellent communication, facilitation, criticalthinking and conflict resolution skills
  • Demonstrated excellent interpersonal skills with ability to work collaboratively with all members of the team
  • Ability to provide leadership to projects and accomplish established goals
