The Road to Recovery Program is a comprehensive approach to substance use care and the first of its kind in BC, providing a full continuum of services for patients and their families. You will be at the forefront of this innovative initiative, helping to address BC's worsening toxic drug crisis. The Road to Recovery Program takes a holistic approach to care, focusing on connecting patients with the right care at the moment when they need and want it, rather than waiting on waitlists and missing important windows of opportunity.

As a

Patient Care Manager with the Road to Recovery Program, you will lead a team of interdisciplinary clinicians including peer support workers, doctors, nurses, social workers, and other allied-health professionals to provide culturally-safe care to patients at every phase of their wellness and recovery journey. As a Patient Care Manager, you will play a critical role in ensuring that patients receive after-care planning to ensure ongoing recovery supports, safe and supportive housing. You will be part of a world-leading urban health patient care environment and will have the opportunity to develop your practice in this dynamic and rewarding field.


Reporting to the Program Director or Senior Manager, this key role provides operational leadership and support to interdisciplinary team members in the provision of quality care to patients/residents and their families.

This position maintains responsibility in collaboration with a Physician Leader for the effective and efficient delivery of services within specified sub-units of a clinical program.

This role promotes and facilitates the continuum of care delivery both internally and with external agencies in alignment with the Program goals and strategic direction by implementing and using quality improvement, utilization management, staff development and resource management strategies.

This position works collaboratively within the Program Based Care Model and in concert with the Program Leaders, other Managers, Practice Leaders, Corporate and Support Leaders.

Qualifications / Skills and Education:
Education


Master's degree in a related Health Profession plus a minimum of five (5) years' experience at the middle management level in a medium to large health care organization or an equivalent combination of education and experience.
