    Description

    Case Manager – Triveri

    This role provides direct support to tenants, ensuring financial assistance is in place and access to health services, mental health care, food bank assistance, community resources and adult education programs. The Case Manager has a thorough knowledge of local resources and relationships with service providers.

    They act as "systems navigators", identifying tenant needs and creating personalized plans for accessing required supports and services. As advocates for the tenants, they facilitate successful receipt of available benefits.

    • Facilitating meetings
    • Providing in-house workshops/training

    The ideal candidate will have experience working with vulnerable populations including homeless individuals or youth at risk. Knowledge of trauma-informed care practices such as crisis management/responses to medical/mental health challenges/emergencies/hygienic practices would be beneficial.
