One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.

We are currently seeking a 1 Full time equivalent Indigenous Systems Navigator who, as a valuable member of the Community Health Centre team, will provide services within the System Navigator scope in the provision of safer supply and primary care support for individuals receiving safer supply services.

There is a strong emphasis on teamwork, system navigation, harm reduction, disease prevention, health promotion and education and community partnerships.


QUALIFICATIONS:


  • Bachelor's degree in Social Work (HBSW), or equivalent relevant experience.
  • Experience working with Indigenous populations and organizations.
  • Ability to effectively engage clients in assessment of needs and goal planning.
  • Knowledge of and connections with community services and partners, including but not limited to health and social services, the judicial system and Indigenous organizations.
  • Demonstrated ability to facilitate and advocate for services to support clients.
  • Knowledge and background in the areas of traumainformed care, harm reduction, culturally safer care, and working with individuals who use substances.
  • Knowledge and proficiency in crisis management and support.
  • Demonstrated ability to work independently and as part of a multidisciplinary team.
  • Sensitivity to the needs of a multicultural community and the impacts of social, economic and cultural issues on health and wellbeing.
  • Respects and values the diversity of communities and individuals.
  • Awareness of and sensitivity to health issues particularly within individuals who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, noninsured).
  • Strong interpersonal, organizational and group facilitation skills.
  • Computer literacy in Microsoft Office.
  • A current driver's license and access to a vehicle to travel to designated outreach sites.
