Monday-Friday 8:30 a.m p.m. (flexibility required to meet program and client needs)

Reports to:
Manager of Community and Clinical Services


Job Overview:

To oversee case planning and provide comprehensive supports to their specific caseload while ensuring implementation of agency philosophy, policy, and procedures.

The Housing Stability Worker, Case Manager supports youth in their successful transition out of the child welfare system into adulthood.

The workers will help youth to develop and pursue their goals, and will support them to identify, access, and navigate adult service systems relevant to their specific needs.


Job Responsibilities and Duties:


  • Providing youth with a continuum of care that includes case management, referrals and outreach
  • Liaising with relevant agencies and programs such as, Youth Justice, Family and Children's Services, Wyndham House, and the Canadian Mental Health Association to provide coordinated care
  • Complete intake assessments to gather a client's background information, and assist them with creating a new successful course of action tailored to their specific needs
  • Provide crisis intervention and support to clients in the program
  • Provide support to staff when clients are in Wyndham House programs
  • Write case notes, intervention plans, treatment plans
  • Plan case conferences and review meetings with relevant community partners
  • Support with data collection and reporting to funders
  • Attend committee meetings and conduct presentations within the community as required
  • Support life skills development
  • Coordinate clients' access to community services
  • Collaborate with community partners to provide wraparound support
  • Provide or support programming related to skills development, recreation and therapeutic goals
  • Provide traumainformed and clientcentred care
  • Design and implement programming to facilitate life skills development
  • Identify community services and resources that are appropriate for clients' needs, coordinating referrals, intakes, appointments, and ongoing access to services
  • Support youth through their involvement with the Youth Justice System, including court, lawyers, mental health diversions, and probation
  • Develop relationships with clients to support emotion regulation, harm reduction, and life skills development
  • Liaise with staff to coordinate care and routines and support with behaviour management and treatment goals.
  • Occasional coverage support to staff sites
  • Provide oncall support 23 weeks per year
  • Other duties outlined from your manager

Job Skills and Qualifications:


  • Post-Secondary education in a Social Services or related field (BSW preferred)
  • 2+ years experience working with youth with complex needs
  • 1 Year experience providing case management and/or support to vulnerable youth.
  • Sound knowledge of the barriers facing streetinvolved youth and commitment to the principles of housingfirst.
  • Working knowledge of the Mental Health Act, Youth Justice Act, and the Child and Family Services Act as they pertain to work with highrisk youth.
  • Skills, knowledge, and attitude consistent with traumainformed clientcentred advocacy and care.
  • Demonstrated crisis management, conflict resolution, problem solving and priority setting abilities.
  • Demonstrated knowledge of local social services and the processes by which to engage them.
  • Demonstrated understanding and sensitivity to social and cultural diversity, including ability to support clients' decisions without judgement or bias.
  • Strong organizational, communication and interpersonal, and crisis intervention skills.
  • Strong ability to problem solve and work independently
  • Background facilitating skill development training or recreational coordination would be considered an asset.
  • Experience with staff supervision and support or program coordination is considered an asset.
  • Crisis Intervention and/or A.S.I.S.T. training would be an advantage.

Mandatory Requirements:


  • CPR / First Aid level C
  • Criminal Reference Check, Including Vulnerable Persons
  • Valid Driver's License, Including 2 million Dollar Liability

Preferred:


  • CYW diploma/ or related degree strongly preferred
  • Bachelor's degree in Social Work or a Registered Social Worker or working towards
