Reporting to the program manager, the Family Support Worker strengthens the role of caregivers of individuals living with mental illness, through supportive counseling, education, the promotion of self-care strategies, assistance with navigating the mental health system and equipping family members to advocate on behalf of their loved ones.

The responsibilities of this position include, but are not limited to:


Responsibilities:


  • Assist individuals and families to identify their concerns, increase awareness of and access to resources and appropriate educational materials;
  • Facilitate individual and group counselling sessions for family members;
  • Support caregivers in building healthy relationships, enhancing personal connections, implementing selfcare strategies, becoming strong advocates for their loved ones and in developing a sense of belonging and support in their community;
  • Initiate intakes and conduct needs assessments;
  • Refer and link clients and caregivers to a range of community resources, including housing, legal supports, health supports, etc.;
  • Maintain client records, case notes and other supportive documentation related to individual support;
  • Maintain client and service data and adhere to all reporting requirements and service evaluation timelines.
  • Relevant undergraduate degree or diploma in social work, counselling, psychology or a related field;
  • Bachelor of Social Work or Master of Social Work will be considered an asset;
  • Minimum 3 years experience providing supportive counselling to individuals and groups;
  • Minimum 3 years experience working with community health or social service organizations, with a emphasis on Community-Based Mental Health;
  • Experience working from a nonjudgmental, family centric empowerment framework;
  • Experience facilitating support groups for adults;
  • Have working knowledge of the Ontario Mental Health Act and other relevant Acts as they pertain to this position.

Key skills and abilities we are looking for:

  • Excellent verbal and written communications skills;
  • Demonstrated counselling and referral skills; and ability to effectively link clients to supportive services;
  • Demonstrated group facilitation skills;
  • Knowledge of and sensitivity to issues of gender, race, sexual orientation and issues impacting diverse communities;
  • Selfdirected and independent, but also experienced in working effectively as a member a team, which includes internal and external stakeholders;
  • Applied computer skills (Microsoft Office, Client Information Systems, Web browsing);
  • Fluency in a second language is a strong asset and preferred;
  • Valid Driver's license and access to a personal vehicle is required.

  • Criminal background check;
  • Must be willing to work flexible hours including evenings, as required.
