    Description

    Your Opportunity:

    The Alberta Children's Hospital is a warm & vibrant workplace with team members, youth, families, staff & physicians partnering together to provide service to the children and families in inpatient units & ambulatory clinics. Reporting to the manager of the Ambulatory Complex Care Service at ACH, the Care Coordinator is an integral member of the Integrated Complex Care Program, providing collaborative family-centered care to children with complex medical needs. The Care Coordinator supports the needs of the patient and family to facilitate holistic and integrated care for their specific social situation and community needs while working closely with collaborating care providers. The team empowers families to access services and supports that enhance their health, well-being, and quality of life in all aspects of their lives. The Care Coordinator will work directly with families who are accessing the Complex Care Program at ACH; this includes facilitating connections to community services and programs, supporting coordination of medical and clinic appointments, education needs, and fostering relationships with community and family. Extensive knowledge of social and community support programs focused on children with complex healthcare need is essential. Knowledge of federal (including those serving Indigenous persons and mental health supports) provincial, regional and local resources is critical to this role.

    Description:

    As a Program Facilitator, you will be responsible for the coordination of services, in addition to operational and clinical leadership. You will support the team to ensure that the services offered are coordinated, effective, evidence based and patient and family centered. Leadership on department/program/service committees is expected. You may also provide varying degrees of direct clinical care at an advanced level. You will provide direction for program evaluation and development within the service or program, and ensure that systems are in place to promote ease of transition between programs internally and externally. You will provide clinical supervision and education to interdisciplinary staff and students in order to support their ongoing day to day professional practice.

    Required Qualifications:

    Master's degree in Social Work, Psychology, Nursing, Occupational Therapy, Education, Physiotherapy, or Speech-Language Pathology or equivalent. Active or eligible for registration with the applicable Alberta regulatory college.

    Additional Required Qualifications: Preferred Qualifications:

    Demonstrated ability to manage change. Excellent analytical and evidence-based decision-making skills. Strong track record in establishing and maintaining effective collaborative working relationships with internal and external stakeholders. Task & Goal Oriented; able to develop and execute a project plan through to completion.
