Job Summary:

Assists person(s) with their physical, economic, vocational, recreational, social, emotional and daily life skills development. Assists clients to achieve the greatest degree of independence and quality of life possible. This position is open to male and female applicants and requires union membership.


Qualifications:
Education, Training & Experience


  • Certificate in related human/social service field
  • One (1) year of recent related experience.
  • Or an equivalent combination of education, training, and experience.
  • First Aid Level 1(OFA) required

Key duties and responsibilities:


  • Participates in collaboration with the Resource Team in the assessment, goal setting and program planning (Personal Service Plan) for person(s) being supported
  • Documents, implements, and provides input into the evaluation of the program and suggests changes to the Program Manager
  • Evaluates client needs and develops short term goals and objectives to meet such needs with the active participation of the person and support network
  • Assists person(s) to function more independently in their own homes and in the community.
  • Provides life skills training and assistance that that aligns with the persons preferences and goals. This may include but is not limited to emotional regulation, alternate behaviours in stressful situations, meal planning & preparation, personal care skills and budgeting
  • Facilitates and provides direct support during person-centered physical, recreational, educational, social and vocational activities.
  • Provides flexible and customized support to person(s) to live in their own homes and participate in meaningful activities in their community
  • Recognizes, analyzes, and takes a pro-active approach to potential emergency situations, using crisis intervention training and strategies to ensure no harm comes to the person, staff and/or the public. Reports crisis to supervisor and completes and submits a Critical Incident form.
  • Administers medications to person(s) in accordance with established policies and CARF standardsJuly 2021
  • Ensures health and safety standards are maintained.
  • Accompanies and/or transports clients to activities such as appointments, shopping or leisure activities.
  • Completes and maintains all required documentation in accordance with established policies and procedures
  • Identifies social, economic, recreational, physical, vocational and educational services in the community that will meet clients' needs.
  • At the direction of the Program Manager, liaison with other agencies, professionals, government officials and the community.
  • Performs other related duties as required.

From $25.05 per hour

Education:


  • DCS / DEC (preferred)

Experience:

Support Care: 2 years (preferred)


Licence/Certification:

  • Occupational First Aid Level 1 (preferred)
  • BC Class 5 Driver's License (required)
