The Family Health Team, Pharmacotherapy Specialist is an integral member of the Pharmacy department and the Family Health Team (FHT).

A key player to advance the pharmacist's role in the clinical care of patients as well as increased patient safety, teaching of pharmacy residents, Pharm D students and mentoring other staff members.

The FHT Pharmacotherapy Specialist will focus their time on direct patient activities.

Secondary, will be to work with the FHT and the Pharmacy Department to establish guidelines and provide implementation strategies for initiatives including:
education to clinical hospital staff, prospective and retrospective evaluation of ongoing pharmacist activities, and provide feedback.

As a leader, you will be responsible for providing education and feedback to students, pharmacists and other members of the healthcare team regarding patient care, therapeutics, teaching and research initiatives.


Duties

  • Conducts comprehensive assessments for individual patients to identify actual and potential drug therapy issues Formulates comprehensive care plans which incorporate individual patient preferences and goals
  • Acts as a clinical pharmacy resource for internal and external health care professionals in a patientfocused practice
  • Work within and Interprofessional team to promote rational evidence based prescribing to optimize patient outcomes within the interprofessional teams
  • Provide clinical direction on complex cases related to their clinical practice
  • Provides teaching on pharmacotherapy to the FHT Staff, trainees, residents, and other Allied Health Staff:
  • Establishes initiatives that foster improved clinical pharmacist activities
  • Establishes initiatives that increase patient safety and promote cost savings
  • Participates in the development of evidence based practice treatment guidelines
  • Manages patients on warfarin through a medical directive
  • Participates in the implementation of the guidelines including:
  • Education
  • Audit and feedback on usage
  • Feedback on patient risk reduction and outcomes as appropriate
  • Patient Safety
  • Provides Drug Information
  • Patient specific recommendations
  • Interprofessional consults
  • Participates in quality improvement initiatives with the goal of improving patient safety
  • Supports opportunities for pharmacybased and interprofessional research within his/her area of practice and specialization
  • Supports hypotheses and research processes, oversees and supports ongoing research activities and/or coordinates the involvement of research assistants, students, pharmacy residents and pharmacists in research activities
  • Collaborates with interdisciplinary teams to conduct pharmacotherapy based research
  • Provides supervision and mentoring for pharmacy students, residents and fellows research projects
  • Publishes in peer reviewed journal
  • Represents the hospital and the University at provincial, national and international meetings
  • Participates in quality improvement initiatives which address the 6 domains of quality in healthcare (Patient Centered, Safety, Timely and accessible, Effective, Efficient, Equitable)
  • Participates in Patient Safety Rounds as opportunity arises
  • Participates in the incident reporting system
  • Represents the Department of Pharmacy Services, The Family Health Team and/ or the University (Faculty of Pharmacy or Medicine) on interprofessional care committees, and standing hospital committees
  • May be required to participate in conferences and regional planning groups as it relates to their clinical practice area

Qualifications

  • Completion of a Bachelor of Science Degree in Pharmacy from an accredited program of study or recognized equivalent
  • Certificate of Qualification with the Pharmacy Examining Board of Canada
  • Current registration with the Ontario College of Pharmacists
  • Pharmacy practice in related areas of patient focused or population health, preferred
  • Completion of a post graduate Pharmacy degree (e.g. clinical Master's degree or Doctor of Pharmacy), preferred
Completion of a pharmacy residency program, preferred

  • Four (4) to six (6) years practical or related experience, preferred
  • Three (3) to five (5) years of hospital experience, preferred
  • Has the ability to work on complex problems with various clinical and technical stakeholders to provide innovative and creative solutions to achieve resolution and maintain patient safety
  • Knowledge of research, statistical and analytical methodologies
  • Demonstrates leadership, teaching, and collaborative team skills
  • Practice experience in the clinical specialty area
