About the Role:


The Safe Beds program is a 4-bed 24/7 residential crisis service, providing an alternative to hospitalization or criminal detention for individuals experiencing a crisis related to their substance use and mental health.

Safe Beds staff provide de-escalation and stabilization of the immediate crisis and connection to the services needed to enable safe and stable community living that addresses client health and wellbeing longer term.

Clients may stay at the Safe Beds for up to 30 consecutive days.

The Intensive Case Manager will work as a member of an interdisciplinary team.

They will work rotational 10-hour shifts, including 2-weeks of days followed by 2-weeks of overnights to provide intake, case management and care coordination supports to the Safe Beds clients.

Qualifications:


  • Completion of a College Diploma or University Degree relevant to mental health, addiction, or social services
  • Experience in mental health/concurrent disorders and trauma an asset
  • Experience working with individuals experiencing mental health and/or substance userelated crisis
  • Experience in social service settings with working knowledge of case management systems and planning techniques
  • Previous experience in addictions mandatory
  • Lived experience related to addictions and/or mental health an asset
  • CSC Clearance conducted by PWGS Canada
  • Standard First Aid/CPR Level C
  • Reliable Vehicle, Ontario Driver's license (G2 or G)
  • Must provide proof from Insurance provider of 1) at least $2 million in general liability and 2) have declared scope of vehicle use to Insurance provider
  • Travel within the Wellington Guelph area (as needed)

Role Overview:


Case Management and Coordination of Care

  • Process and accept referrals from Guelph Police Services, Wellington County OPP, and the Guelph Wellington community hospitals
  • Screen and assess referrals for program admittance
  • Conduct intake and enrollment activities
  • Obtain all appropriate consents & authorizations to ensure effective communication with other service providers providing linkages, resources, referrals, etc.
  • Conduct intake and enrollment activities
  • Refer and link ineligible referrals to appropriate services and linkages
  • Support the development of comprehensive discharge plans, including access to appropriate shelter/housing and process program discharges
  • Create and support implementation of individualized care plans including access to health care, entitlements, life skills, community integration, and treatment as appropriate in collaboration with community partners
  • Create and implement crisis response plans
  • Support clients to access primary health, and other health and social services provided internally or by community partners
  • Maintain appropriate confidentiality and documentation standards as required by the agency

Program and Client Support

  • Monitor the Safe Beds house
  • Conduct bed checks
  • Provide medication to clients and complete required documentation
  • Monitor withdrawal symptoms as necessary using withdrawal monitoring scales
  • Participate in program development as needed
  • Some light cooking and housekeeping responsibilities required, including room inspections and turnover upon client discharge, laundry, and sanitization of common spaces

Administration and Reporting

  • Involvement in program evaluation, data collection/input into CaseWORKS
