Position Summary

  • The Couchiching OHT (COHT) Care Clinic
  • Social Worker is responsible for providing assessments, treatments and educational services to patients and families including a broad population scope spanning from children and youth to perinatal mental health to seniors. This includes COHT programs & services. The primary accountability of this position is to provide high quality professional counselling and case management to assigned patients and families, consistent with the philosophy of Orillia Soldiers' Memorial Hospital.
  • PRIMARY RESPONSIBILITIES:
  • Providing comprehensive psychosocial assessments, which identifies patient and family strengths, problem-solving abilities, dynamics, interactions, attitudes, feelings and cultural issues for optimal care delivery.
  • Providing a resource to other staff members and utilizes a researchbased practice.
  • Assessing risk factors for abuse, mental illness and suicide.
  • Networks and Liaison with community agencies.
  • Documenting the social patient care plan. Maintaining accurate records and statistical data.
  • Utilizing a variety of modalities (individual and group) to provide intervention to assist families, such as clinical counselling, consultation, case/crisis management, perinatal mood disorder knowledge and Clinical Education on Various Paediatric Mental Health adjustment to illness and grief process, community resources and patient advocacy.
  • Knowledge of and experience with utilizing a variety of therapeutic techniques including Cognitive Behavioural Therapy (CBT), Solution Focused and Strength Based approaches
  • Participating in program development including policy and procedures.
  • Educating children, adults, and families, students, and health care professionals.
  • Adaptable to meet the changing climate of service delivery as needed.
  • Obtaining and maintaining competency and standards as per the Ontario College of Social Workers and Social Service Workers and remain in good standing with governing body/college.
  • Remain current in?? training specific to populations being supported such as Canadian Perinatal Mental Health Trainings (CPMHT, 6 free modules available)
  • Be aware of and remain up to date on opportunities to implement new, evidencebased methods of patient assessment and treatment.
  • Attend appropriate workshops, professional conferences, in services and participate in eLearning, Journal and literature reviews as required to update knowledge and skills pertaining to diabetes education management, mental health support and other identified areas of the COHT patient population.
  • Participate in clinical projects/studies as required within agreed upon timelines of the team
  • Provide education and mentorship to Social Work Students and Program Team Members.
  • Maintaining proficiency in computer skills.
  • It is a requirement that the Social Worker works a variety of shift types including days, evenings, weekends and holidays.

Qualifications:


  • Education:
  • Current registration with the Ontario College of Social Workers and Social Services Workers required.
  • Masters of Social Work required.

Experience:


  • Minimum three (3) years' work experience in an inpatient setting or the community required.
  • Experience with Children and Adults with medical and mental health illnesses required.
  • Certificate in CBT and PMD training and education required.
  • Ability to work and problem solve independently.
  • Strong interpersonal skills to function as a lead member of the multidisciplinary team.
