St. Mary's Home is a not-for-profit charitable organization serving the Ottawa area and beyond. SMH has continuously and responsively served pregnant youth in Ottawa since 1933.

Specialized residential and community support services are provided to marginalized young pregnant women (ages 12-25), young parents and their children.

The approach is holistic, grounded in a strong commitment to strengthen and build upon the personal resilience of each individual.

The
Bilingual Case Manager reports to the Residential Manager.

The Residential Case Manager (RCM) implements the Mission Statement of SMH by providing Support and Case Management to at-risk pregnant / parenting youth and their infants who are living in our Residential Program.

Within a strengths-based philosophical framework, the RCM promotes positive Residential dynamics, defuses negative dynamics and is a positive role model.


Oversees the Residential stay of residents on their respective case load, advocates for residents and liaises with our Young Parent Outreach Centre and multiple service partners.

Ensures resident files meet annual Licensing standards. Ensures continued excellence in meeting or exceeding clinical and operational standards.


As a member of the Residential Team of SMH, the role includes responsibilities contributing to a comprehensive and seamless service for our residents.

Please note this is for a
Bilingual (French-English) Position.

Responsibilities:


  • Engages the resident oneonone in an effective and positive manner and within a Collaborative Problemsolving framework
  • Oversees resident crisis management
  • Oversees residential dynamics within a Crisis Prevention Intervention (CPI) framework
  • Facilitates emergency intakes and admissions and oversees waitlist
  • Implements resident assessments using approved evidencebased tool
  • Provides support for residents in developing and meeting their Care Plan goals
  • Oversees resident interface with Child Welfare, Public Health and other Community Services
  • Responds to resident mental health and addiction recovery, family violence and traumarelated issues
  • Provides support for residents with Conflict Resolution, Crisis Support and Residential Guideline Compliance
  • Watches over Bedroom Standards, assisting residents with handson support to meet these requirements for fire, health, safety and licensing
  • Assists residents in meeting their attendance requirements for appointments and programs
  • Oversees residential discharge planning and postdischarge linkages
  • Records in residential files to include electronic file management system
  • Projects a positive image of SMH in the community

Participates actively in staff meetings:


  • Provides coaching with Primary Care Staff on Collaborative Problem Solving and Conflict
  • Resolution skills building
  • Supports an integrative approach within the agency between the Residence and the Young Parent Outreach Centre
  • Engages in regular supervision and support
  • Covers when fellow CM is not available
  • Promotes positive morale among team members and agency wellbeing

Qualifications:


  • Bachelor's degree in Social Work or related clinical equivalent
  • 4 years counseling / social work experience directly related to addictions, mental health, family violence and/or trauma issues and infant mental health.

OR

  • Diploma as an CYW, SSE or related equivalent
  • 2 years of progressive experience working directly related to at risk youth, mental health, family violence, trauma and infant mental health
  • Experience in a Residential Setting an asset
  • Minimum 3 years experience working with pregnant and parenting youth / At risk youth.

Required Knowledge and Skills:


  • Excellent communication and interpersonal skills
  • Ability to work independently and also as a member of a team
  • Capacity to provide service in both official languages is strongly preferred
  • Experience with Motivational interviewing
  • Certificate and applied knowledge of Crisis Prevention Intervention
  • Knowledge of adolescent pregnancy and parenting issues
  • Knowledge of Infant Mental Health and Attachment is a strong asset
  • Certification with the GAIN SS and CANS Assessment tools is an asset, but can be acquired once in the position
  • Compassion, enthusiasm and the ability to take initiative
