    JOB PURPOSE

    To utilize a case management approach in the development and co-ordination of the Bleeding Disorder (Haemophilia/ Benign Haematology Disorders) Clinic, providing quality nursing care for patients living with bleeding disorders and other hematologic conditions in the province of New Brunswick. Care delivery includes clinic settings, in-patients, and the community. This clinic position will provide education, counselling, clinical and follow-up services for individuals and families of those with bleeding disorders and other haematological conditions.

    QUALIFICATIONS

    • Bachelor of Nursing degree
    • Current registration with the Nurses Association of New Brunswick
    • Current certification in CPR.
    • Must have recent (last 2 years) knowledge and expertise in bleeding and haematological disorders care, treatment protocols and best practice care standards for this patient population.
    • Ability to perform venipuncture, initiate peripheral IV access and provide care for venous access devices (or willingness to obtain skill set within 2 months).
    • Valid driver's license and willingness to travel within NB.
    • Minimum of 5 years of recent clinical experience in medical/surgical nursing
    • Demonstrates initiative and effective communication and organizational skills.
    • The successful applicant shall be constantly motivated to keep up to date on new research developments and be willing to attend medical and educational conferences on Haemophilia.
    • Ability to attend work on a regular basis.
    • Ability to perform the duties of the position.
    • Good work record.

    KEY RESPONSIBILITIES

    Clinical Practice

    • Function as a mentor, support person and educator for all healthcare providers caring for in-patients both adult and paediatric.
    • Collaborates and coordinates with the patients' healthcare providers the ordering and administration of Factor products and laboratory tests.
    • Developing treatment protocols and guidelines in collaboration with Haematologists.
    • Collaborating effectively with other Bleeding Disorder Nurses in the province, while providing a provincial On Call Service to ensure shared 24/7 coverage.
    • Knowledge expertise in etiology of bleeding disorders, blood products, and coagulation basics, bleeding symptoms, psycho-social issues, assessment, and treatment of complications such as pain and joint swelling.
    • Knowledge skill set of venous access devices and guidelines for usage.
    • Works collaboratively with the clinic physicians and bleeding disorder team in the assessment and planning of care to meet the individualized patient's needs.
    • To co-ordinate and follow up on the diagnostic evaluations and tests that are needed.
    • To assist the Haematologist in designing treatment protocols for management of common problems such as musculoskeletal support needs and bleeding, or to establish protocols to infuse patients who are unable to self-infuse.
    • Facilitate treatment follow up (referrals made, physician orders complete and transfer of care to family physicians communicated)
    • To support supervised self-therapy by encouraging patient and family education; supervision and organising self-treatment; observing, recording, and anticipating problems; monitor factor replacement and to reinforce educational programs.
    • To organize professional counselling in genetics, financial impacts, and psychosocial problems, educational/vocational needs, physical and recreational needs when needed
    • Mentor and educate staff, patients and support persons in the preparation and administration of blood products and other IV medication.

    Communication and Outreach Service

    • Responsible for: the follow-up of all patients and to arrange care in the local health facility, maintain contact with patient to ensure treatment compliance, and to maintain regular and on-going contacts between patient, family, and centre. The co-ordinator will need to make home and school visits when indicated to support patient needs.
    • The development and provision of educational programs that better prepares the bleeding disorder patient to deal with the challenges associated with their condition.
    • Develop a health education program that provides patients, family, community, and health professionals with information that includes the basic information about bleeding disorders, specific sites of bleeding and sequelae, supervised self-therapy, psychological aspects, genetics, and dental problems. Provide in-services to medical staff as required.
    • Co-ordinating care and acting as a liaison between the patient, the clinic and the community.
    • Acts as a liaison between the Haematologists and Program Managers, patient, family, other health care professionals, and the community.
    • Maintain membership in the Canadian Association of Nurses in Hemophilia Care (CANHC).
    • Maintain contact with the public and related support organizations on a local, provincial, or national level.
    • Participate in a yearly camp for children with bleeding disorders.

    Administrative

    • Collecting and maintaining accurate monthly clinical records (including CBDR), statistical data and preparing monthly reports on clinic activity.
    • Communicate with regional Blood banks on product availability.
    • Ensuring mailouts/ communications are sent to patients in a timely manner.
    • Ensure communications are sent out to members of the patients' health care team.

    Research

    • Policy and protocol development with yearly review and update.
    • Participate in clinical trials in Bleeding Disorders as required; including educating the patient and family/caregivers on the issues related to clinical research and assess patients for appropriateness of study participation.
    • Promotes and implements the use of valid and current research findings into policies, procedures, and standards of Bleeding Disorder nursing care.
