    Title: Registered Practical Nurse

    Location: Hope Home, ARISS Wellington Rd 86.

    Positions Available: Full-Time, Part-Time, and Casual

    Hopewell is a not-for-profit registered charity governed by a volunteer Board of Directors. We were established in Ariss, Ontario in 1983 by John and Johanna Oosterhuis.

    Hopewell provides high-quality personal care for children, youth and adults with developmental disabilities and complex needs, as well as support for their families.

    Our unique culture and well-deserved reputation for extraordinary staff and exceptional care have allowed Hopewell to expand to seven locations, meeting the needs of over 500 families throughout Wellington County.

    JOB SUMMARY

    Under the guidance of the Clinical Nurse Manager, the Nurse works as a professional within a clinical multidisciplinary team that follows a biopsychosocial model of intervention for children, youth and adults with complex developmental, physical and emotional care needs. The Nurse works alongside of the Direct Care Support Worker to assist in maintaining high quality supports for all clients. The Nurse is primarily responsible for supporting the health care support needs of clients.

    Duties and responsibilities

    Performs a variety of interventions as prescribed for the supported individual in a manner that ensures supported individuals' dignity, respect and confidentiality:

    Assessment

    • Provides initial and ongoing assessment and interpretation of supported individual within the framework of holistic professional nursing practice, and in accordance with the principles of person-centered care;
    • Assesses and interprets information and data regarding the supported individuals' culture and values system, lifestyle, and interest and ability to participate in their care, and other factors impacting successful treatment outcomes;
    • Identifies and communicates individual needs to the interdisciplinary care team and other providers of care as appropriate;
    • Provides ongoing monitoring of individuals response to intervention; identifies deviations from expected outcomes and assesses change in supported individuals' status.

    Intervention

    • Establishes and maintains a therapeutic relationship with supported individual and families;
    • Identifies, initiates and coordinates therapeutic interventions consistent with the legislated scope of practice and level of competence;
    • Implements prescribed interventions (e.g. diagnostic tests, medications) and performs independent nursing interventions (e.g. individual and family education).
    • Ensures the implementation, coordination and ongoing evaluation of the supported individual's care plan;
    • Consults with the interdisciplinary team and other care providers (including Senior Behaviour Therapist, Doctor's, Dentists etc.) to discuss changes related to individual's status, and modifies interventions as needed;
    • Interprets and provides medical information including diagnosis and treatment plans to supported individual and their family and responds questions and concerns;
    • Provides information and instruction to assist supported individual and family to understand health care practices and processes.
    • Utilizes educational material, appropriate to the capacity of the supported individual, to assist the individuals understanding of their health care, health promotion, and/or health maintenance, and the enhancement of their individual well-being.
    • Care is provided alongside Direct Care Support Workers to ensure a consistent and coordinated approach.
    • Responsible for implementing instructions as directed by specialists and consultants, including medical professionals and behavioural therapists.
      1. Updates supported individuals' charts/forms, monitors vital signs, performs venipuncture, administers feeding and personal care plans, and all necessary Hopewell documentation etc;
      2. Checks medication profile and dispenses medications to assigned residents and documents medications on the profile as given;
      3. Monitors supported individuals behaviour, reaction to treatment and responses, reporting any unusual incidents or concerns to the Clinical Nurse Manager;
      4. Perform health acts with appropriate training, as assigned by nursing staff including but not limited to medication administration, working with G-Tubes, trach tubes, wound care, etc;
    • Demonstrates competence (knowledge/skills/attitude) in the use of non-violent physical crisis interventions, trauma-informed care and other best practices available to promote healing and recovery.
    • Maintain awareness of advances in client services, government regulations:
      1. Possess and demonstrate knowledge of First Aid, CPR, CPI, WHMIS, Occupational Health and Safety Act, fire prevention and safety procedures and other areas as deemed necessary by the Hopewell;
      2. Ensure that Health and Safety, Serious Incidents/Occurrences and Behaviour Support Plans are understood, followed and reported;
      3. Provide hands on training for medication administration, personal care, etc. to other team members as required.
    • Immediately advise the Clinical Nurse Manager and/or management team of issues/problems/concerns regarding client care issues, staff, buildings, equipment and vehicles.
    • Provide training within and outside of Hopewell to facilitate educational seminars and training sessions as assigned.
    • Attending medical/specialist's appointments alongside supported individuals as required.

    Accountability

    • Practices within professional, legal and ethical standards and is accountable for his or her own actions and decisions at all times;
    • Develops cultural competence in order to provide culturally competent care;
    • Actively participates in developing and evaluating professional goals and performance at regular intervals (e.g. performance management process);
    • Role models the principles of continuous quality improvement by critical reflection of own practice, seeking meaningful feedback from peers, supported individuals and family/guardians and managers;
    • Maintain your registration and remain in good standing with the College of Nurses of Ontario;
    • Maintain knowledge of current best practices in the field of nursing, including accessing educational opportunities;
    • Accountable for reporting professional practice issues according to legislation and standards of practice to the appropriate person, agency or professional body.

    Complete documentation and training, this includes:

    • Completing necessary paperwork on shift, with high levels of data accuracy and attention to detail;
    • Participating in ongoing training and development and ensure all mandatory training requirements are met and maintained.

    Maintain healthy internal relationships, this includes:

    • Recognizing and responding to vocalized and implied needs of residents in a timely, nurturing and professional manner, as well as working well with workplace peers/colleagues;
    • Maintaining a positive attitude about the workplace is critical to maintain a pleasant relationship with residents and peers;
    • Directly reporting potential/actual areas of concern about residents/peers to their team leader or manager.

    Special circumstances

    • Work flexible hours within a variety of settings as required by the changing needs of the program and the people we support;
    • When required, must be able to undertake lifting of people following guidelines for proper back care, lifting and transferring as required by the program;
    • Diploma in Practical Nursing or Bachelor of Science, Nursing;
    • Member of the Ontario College of Nurses in Good standing;
    • Must have valid class G driver's license.
