- Develop, implement, and coordinate the case management process.
- Gather information from assessments and Participants meetings to identify barriers to housing; develop and implement a Participant care plan outlining goals and steps taken to achieve them.
- Help Participants to identify goals, continually monitor and evaluate these goals, and implement discharge plans for Participants that have obtained housing.
