Hospice Palliative Care Coordinator

1 Full time, or 2 part time job share.

By providing quality compassionate care that supports the physical, emotional, spiritual and psychosocial needs of individuals and families coping with the realities of a palliative illness, we're helping to make people's lives better.

We're looking for a Hospice Palliative Care Coordinator who shares our passion for care to join our team.


As a member of the DLH Community Team you will support individuals and those close to them navigate health care services and develop personalized care plans based on individual needs, preferences, hopes and goals.

Individual-centered care ensures respect, dignity and choice.

As our Hospice Palliative Care Coordinator you will work closely with colleagues in Home and Community Care Support Services, the DLH Community Physician Team, and the DLH Community Team to provide holistic end of life care in the home.


Environment:

In the community (home setting of persons served), may include other health care settings; and at The Dorothy Ley Hospice.


Position:
One Full-time permanent position; OR Two part-time positions as a job share


Key functions of the role include:

  • Provide counselling and psychosocial support to individuals at end-of-life and their families. Foster a rapport that enables the individual and their family to explore sensitive issues present when facing the challenges of a life-limiting illness
  • Provide education and information about end of life tailored to each individual and family
  • Contribute to the care of the individual through systems navigation and advocacy
  • Coordinate access to a broad range of support services based on the person's needs
  • Conduct assessment of the individual's needs for services and support and collaborate with the individual and their family to develop a care plan reflective of the identified needs and goals
  • Implement the care plan by linking the individual and family to Hospice services (including spiritual care, wellness services, inhome volunteers, caregiver support and residential care)
  • Build and maintain relationships with community groups and health care partners.
  • Work with community groups and the Manager, Community Programs to develop or modify hospice palliative care programming to be relevant and appropriate for diverse minority groups.
  • Support outreach activities to educate community members and health care providers about hospice palliative care and the services provided by the Dorothy Ley Hospice.

Reporting to:
Manager, Community Program.


Qualifications:


  • Bachelor of Social Work degree, RPN or RN with 3 years palliative experience.
  • A member in good standing with the applicable regulatory college
  • Minimum three years' experience delivering palliative care
  • Experience working in a community setting
  • Experience in providing traumainformed care an asset
  • Evidence of a dedication to continuing education.

Position Competencies:


  • Experience providing Hospice Palliative Care
  • Comfortable working with individuals with a life limiting illness and able to initiate and facilitate discussions addressing issues related to palliative illness
  • Knowledge of community resources relevant to hospice care
  • Excellent interpersonal and partnership skills
  • Excellent active listening, oral and written communication skills
  • Ability to work in a selfdirected capacity
  • Committed to protecting the privacy and confidentiality of persons served
  • Interest and experience in working with health care partners including volunteers
  • Proven time management and organizational skills
  • Proficiency with Microsoft Office Suite and data tracking
  • Experienced in maintaining client records

Position Requirements:


  • A Police Vulnerable Sector check is required as part of employment
  • Ability and comfort with inhome visiting is central to the position
  • Valid driver's license and access to a reliable vehicle, with insurance covering driving for business purposes.

$62,000.00-$80,000.00 per year
