What will be your role?


The Scheduling Coordinator is a unique integrated coordinator role at VHA Home HealthCare and responsible for the scheduling and information exchange for personal support, nursing and rehab professionals as part of this innovative East Toronto Health Partners' (ETHP) strategy to improve client transitions from hospital-to-home through the newly established MGH2Home program.


Reporting to the MGH2Home Program Supervisor, the Scheduling Coordinator supports rapid transitions of clients and families by communicating effectively with our clients, hospital partner and health care providers to enable effective service utilization in this unique partner program involving VHA, WoodGreen and Spectrum.

This is a fast-paced program, and the part-time Scheduling Coordinator is critical to supporting clients and families who rely on care 7 days per week.

This short-term transitional program (up to 90 days) involves linking together all of our partners to support earlier discharges of adults and provide needs'-based care which is continually evaluated.

The goal is to successfully transition clients with optimal improvement where possible. back to their home and community.

This multi-disciplinary team will work as "one team", along with the client and designated family, to address their complex health and social needs in the development of a single "one team" care plan.


Each work day may involve preparing for new client referrals, communication with clients and families, initiating and updating interdisciplinary service plans, responding to scheduling requests and changes and regular communications funders, Personal Support Workers, Nurses, and Rehabilitation Service Providers as required.

The Scheduling Coordinator will work closely with other partner organizations who are part of the MGH2Home program and ensure ongoing communication with partners, front-line Service Providers, home and community care support services, and other community services.

The Scheduling Coordinator is also responsible for inputting accurate information for data collection purposes for the program.

Data input to support Power BI dashboards, effective documentation and advance planning are key components of this role as well as responding to MGH Transitional Navigators working on the weekends for transition planning.

Required Qualifications:


  • Post-

Secondary School Education:
a certificate/diploma in healthcare education is preferred

  • Minimum of three years related work experience in a healthcare organization/agency (preferably in the community) or in an administrative position that required extensive scheduling; experience of multidisciplinary scheduling health care providers in the community sector would be an asset
  • Empathybased and personcentered telephone communication skills with the ability to actively listen and to respond effectively to crucial conversations that require escalation to a supervisor/manager
  • Strong organizational skills with attention to detail and the ability to prioritize multitasking and quickly adjust to change
  • Excellent communication and interpersonal skills
  • Experience working within an interprofessional or integrated care approach is an asset
