GENERAL DESCRIPTION:
The Client Care Coordinator is responsible for coordinating HSW/PSW and client schedules and associated tasks according to client requirements and in accordance with TNG operating standards, the collective agreement and funder requirements.
Scheduled hours of work may include evenings, weekends and statutory Holidays. This position reports to Senior Manager, Supportive Housing and is part ofCUPE Local 7797.
MAJOR DUTIES:
- Develop and maintain a coordinated Master Schedule for Home/Personal Support Workers and clients within a complex, high needs environment and in a manner that is responsive to the immediacy/urgency of supportive housing clients.
- Conduct home visits/assessment as needed with clients and/or caregivers to ensure that services are appropriate and are being appropriately provided.
- Develop/update Care Plans for clients.
- Inform all relevant staff of developments relating to clients
- Participate in and/or facilitate and take notes of team meetings as required
- Prepare weekly meal plan for JDH. Keep accurate inventory of food and supplies. Organize and place grocery order for JDH.
- Liaise with other social and allied Health Care Professionals to coordinate client health/psycho/social needs
- Document in Alayacare and/or report directly to the appropriate Supervisor, in a timely manner, all communications with H/PSWs, clients, caregivers or Health Care Professionals that could impact client care and wellbeing.
- Adherence to TNG Policies & Procedures, Codes of Conduct, Confidentiality, Ethics, Non-Discriminatory and Risk
- Perform other duties, appropriate to the position, as required
REQUIREMENTS & QUALIFICATIONS:
- Strong, proven scheduling experience in a complex environment using the Alayacare system.
- Training and proven experience in MS Office (Word, Excel), and Outlook are required.
- High degree of accuracy and efficiency in data entry.
- Experience and knowledge of issues affecting seniors such as dementia, palliative care, stroke, mental health, disability and health access is preferred.
- Working knowledge of local program and service resources.
- HSW/PSW related experience complemented by jobrelated education and training.
- Proven interpersonal, organizational, written and verbal communication skills.
- Demonstrated selfstarter with experience and commitment to a team approach in the workplace.
- Ability to work independently in a fast paced, diverse and multicultural environment.
- Knowledge and experience in implementing antiracism and nondiscriminatory practices.
- Familiarity with the HCCSS referral process an asset.
- A valid Ontario Drivers License is preferred.
- Assessment and/or Inter-RaiCHA training an asset
Salary:
$30.34 per hour
