Reporting to the Manager of Integrated Services,
the incumbent will be responsible for the following functions:

  • Providing client needs identification through ongoing assessment and client directed care planning
  • Implementing client centered plan of care that focus on strengths, wellness and empowerment to support recovery and goal achievement
  • Providing crisis prevention and intervention planning with the hope of preventing a relapse and/or hospitalization
  • Acting as a resource person or an advocate to assist individuals in obtaining information/access to services/supports which are deemed needed
  • Liaising with family/significant other and natural supports to facilitate the clients' links within the community

Qualifications:


  • Postsecondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W; a Registered Nurse or Registered Practical Nurse with current registration with the C.NO. (preference to those with Canadian Certification in Psychiatric/Mental Health Nursing); Bachelor's Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W; or a postsecondary degree with a relevant professional registration (acceptable to the employer);
  • Experience and a detailed knowledge of serious mental illness, usually gained by several years of working with this population
  • Valid Ontario Driver's Licence and access to a vehicle are required

Hours of Work: 35 hours/week. Flexible hours are required to meet the position requirements.


Salary:
Commensurate with experience and education within the range: $53,393 to $75,995 inclusive of HOOPP.
