    Withdrawal Management Services (WMS) provides detoxification, assessment, referral, life skills, relapse prevention, education, and case management services to males and females with substance abuse and associated psycho-social issues in residential, day and community settings. The Case Worker is responsible for providing case management counseling and follow-up treatment programs for clients in the Withdrawal Management Program in the community and liaising with the hospital.

    DUTIES & RESPONSIBILITIES:

    • Meets all new clients and acquaints them with the various options/programs available internally/externally to the Withdrawal Management Service.
    • Provides support to the clients in identifying personal goals and lifestyle changes.
    • Provides counseling for clients and ensures continuity in service while they are awaiting placement in treatment programs.
    • Acts as a liaison with the community agencies and hospital to ensure appropriate client referrals.
    • Networks with community agencies and hospital and acts as a client advocate for the Withdrawal Management Service.
    • Demonstrates excellent ability to develop relationships across the hospital and community agencies
    • Meets all new clients and acquaints them with the various options/programs available internally/externally to the Withdrawal Management Service.
    • Completes Provincial Assessment Tools when required and develops a discharge plan for each group member.
    • Acts as a liaison with the community agencies and hospital to ensure appropriate client referrals.
    • Networks with community agencies and acts as a client advocate for the Withdrawal Management Service and Coordinated Access.
    • Experience and knowledge of community case management in addictions
    • Liaises with primary care providers to ensure services for clients.
    • This position would also provide support and additional coverage to the day program and coordinated access for addictions and also to other community withdrawal caseworkers

    QUALIFICATIONS

    • Two year college diploma in Human Services Counseling, or Social Service Worker or university degree preferred with related program that has a significant portion of its course content in addictions or concurrent disorders
    • Minimum 2 years working in the addictions field
    • Experience working as community addictions caseworker preferred
    • Experience working with clients going through withdrawal
    • Extensive knowledge of Withdrawal Management Services, addictions and social service resources in the community
    • Experience working in a cross-disciplinary team or in a medical environment dealing with addictions
    • Crisis Intervention training
    • CPR/First Aid
    • Excellent written and verbal communications skills
    • Experience working with diverse communities
    • Ability to work independently
    • Competencies in Word, Excel, and PowerPoint
    • Ability to work flexible hours
    • Experience working with clients with concurrent disorders
