The Role and Opportunity:

The Canadian Mental Health Association seeks a Case Manager for the Lurie Housing Program (LHP).

The
Lurie Housing Program provides housing support, 24/7 drop-in support and case management services to individuals with mental illness and/or addictions who have experienced chronic homelessness.

The Lurie Housing Program is partnered with the City of Toronto.

RESPONSIBILITIES


Reporting to the Program Manager, the Case Manager will provide site-based support to individuals with mental illness and/or addictions who have experienced chronic homelessness.

This position will be based out of Steve Lurie House, located on Danforth Road, with some travel to other community resources as required.


The Case Manager will aim to work closely with the Steve Lurie House team and other service providers in the community, link individuals to needed support and services and advocate for the clients on their behalf.

In addition, the team works together using a harm reduction framework to provide ongoing community and site-based services to obtain optimal housing stability for the Steven Lurie House residents.


The role will involve case management duties where the Case Manager builds and maintain a trusting, respectful and collaborative working relationship with clients and colleagues, service providers and other community supports, including client-centred engagement, service provision, interventions, skills development and goal planning.

Salary:
Salary: $49,586 - $61,500 + 4% Lieu in vacation


Work Location: 50 Danforth Road, Scarborough, ON

Work Hours:
Monday - Friday, 8:00 a.m. to 4:00 p.m.
