Home and Community Care Support Services Champlain is one of 14 Home and Community Care Support Services organizations in Ontario.

We are dedicated to ensuring our more than 64,000 patients annually receive the highest-quality health care and services at home.

Our patient-care coordination teams help develop care plans for people of most ages, focused on maintaining each patient's independence and dignity.

We also provide long-term care home placement services, and facilitate our patients' access to the community-based services they need.


Position Summary:


Under the general direction of the Privacy Officer and Manager and in accordance with provincial and Home and Community Care Support Services Champlain ("HCCSS") standards, legislation and guidelines, the Health Information Specialist is responsible for ensuring the privacy and data integrity of health information records.

As well, the Health Information Specialist will focus on retention, storage, management and disposition of health records that are kept primarily in electronic form.


The Health Information Specialist acts as a resource on data integrity to Home and Community Care staff, including both managerial and clerical/administrative staff.

As well, the Health Information Specialist will act as a Privacy resource, including primary responsibility for Access to Information requests under PHIPA and FIPPA.

On a regular basis, this position will provide mentorship and leadership to the Records Management Clerk within the Records Management Department.

This position will monitor and report on the accuracy, efficiency and quality of all Records Management processes and procedures.


Primary Responsibilities:

Quality Management

  • Upon request or through random quality checks, conducts qualitative analysis of client records in client system and on the Document Management System (DMS) by reviewing CHRIS notes. Compares records against the standards established through the Professional Practice Committee of the Champlain LHIN
  • Routinely audits the content of health record notes for adherence to HCCSS Champlain Documentation Standards and abbreviations list for appropriate use of abbreviations in order to support the Health Information Manager in the preparation of reports for Client Services
  • Participates in quality improvement initiatives by assessing proposed guidelines, procedures and processes related to health information management, electronic document filing, for efficiency and effectiveness and making recommendations to managers for improvement.
Privacy

  • Investigates potential privacy breaches under the direction of Manager, Health Information and Privacy Officer
  • Conducts systemic and random privacy audits on local and external electronic patient databases, (eg. CHRIS, Clinical Viewer) and ensuring outcomes are communicated and addressed with the Manager, Privacy and Health Information.
  • Supports the development and implementation of a privacy and information integrity infrastructure
  • Assists in the preparation of a regular report to senior leadership on status of Privacy Integrity within the organization
  • Assists with development and implementation of organizational Privacy Training strategy and program.
Information Management

  • Manages the archiving, retrieval and destruction of paperbased health records, following all defined security and privacy practices
  • Receives, reviews and responds to all requests for release of information from patient records
  • Ensures appropriate consent and/or documentation is in place to proceed with authorized release of records, in accordance with relevant legislation
  • Tracks all requests for release of information, in accordance with HCCSS policy and procedure
  • In collaboration with the Manager, coordinates activities related to Freedom of Information (FIPPA) access requests, according to established Ontario Government legislation and guidelines.
Electronic Health Record

  • Acts as a core resource regarding Documentation Standards and Docushare (DMS) to all HCCSS staff
  • Assesses client services form requirements on an ongoing basis with a focus on best practices and streamlined reporting
  • Demonstrated strong presentation and facilitation skills
Occupational Health and Safety/Infection Prevention and Control

  • Follow safetyrelated policies, procedures, and protocols, including the use of personal protective equipment
  • Participate in training and educational opportunities
  • Demonstrate proactive approach to safety and seek opportunities for improvement by asking questions and making suggestions related to current policies and processes
  • Report safety concerns, errors, and/or near misses that may put staff and/or clients at risk and participate in corrective action
  • Support client education initiatives, such as the Welcome Package
  • Encourage clients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or fee
