Qualifications:

Master's of Social Work Degree required.
Current registration in good standing with OCSWSSW without terms, limits or conditions.
Minimum 2 years practice in social work.
Expertise in case management work with a broad knowledge of family dynamics and work with the disadvantaged and elderly populations.
Demonstrated knowledge and skills in psychosocial assessment, conflict resolution, creative problemsolving and complex discharge planning to other facilities (e.g. longterm care) based in trauma informed care.
Demonstrated knowledge and experience with supporting patients through trauma.
Demonstrated experience with a broad range of patient populations including paediatrics, adults, geriatrics and general medicine patients.
Demonstrated flexibility in responding to patients who present a wide range of psycho social problems and flexible to meet the needs of the Emergency Department.
Demonstrated positive working relationships and collaboration with members of the care team.
Good communication and interpersonal skills and the ability to function as a team player.
Excellent time management skills and demonstrated ability to perform in a fastpaced environment with mínimal supervision.
Satisfactory attendance record.
