We promote integration in the communities surrounding Victoria through partnerships with the individual, their family members and the community, to enhance and enrich the quality of life for people with a mental illness.


Position Summary
The Bridge and Seniors programs offer social groups combined with one-to-one support designed to assist individuals in their recovery.

Groups address various Psychosocial Rehabilitation (PSR) domains, including leisure, basic living skills, wellness, and connection with peers.

Bridge and Seniors helps clients develop meaningful activities, build skills to better understand their mental health, build a support network and transition to community.


Duties

  • Implements programs such as social, wellness, and recreational activities to meet pre-established goals and objectives;
  • Supports clients in the planning and achievement of individual goals, by liaising with community resources, associations and other supports;
  • Support clients to address recovery goals and needs in the areas of basic living skills, wellness, and leisure;
  • Ensures the intervention plan supports a multi-disciplinary team approach to treatment;
  • Document and report Recovery Plans and progress with supervisor and team;
  • Builds constructive working relationships characterized by a high level of acceptance, cooperation and mutual respect;
  • Communicate effectively with client / family and team members;
  • Use a variety of facilitation techniques to achieve desired outcomes;
  • Maintain documentation standards as set forth by the agency, and risk management;
  • Develop a safety plan, as necessary, in response to observed or client-identified concerns regarding safety of the client; in consultation with supervisor
  • Accompanies clients and participates on outings following all ICMH safety procedures;
  • Use research findings to enhance program offerings;
  • Act as an advocate for the advancement of the profession;
  • Facilitate community partnerships;
  • Participates in quality improvement activities and implements changes as necessary;
  • Performs other related duties as assigned.

Qualifications

Education

  • Diploma in Social Services or health care fields or an equivalent combination of education, training, or experience;
  • Basic First Aid and Level C CPR certificates;
  • Food Safe certificate.

Experience

  • Recent, related experience of two years;
Or an equivalent combination of education, training, and experience;
Or other Qualifications determined to be reasonable and relevant to the level of work

  • Knowledge and implementation of evidence based and evidence informed PSR practices as essential tools for recovery;
  • Has knowledge of major types and recoveryenhancing interventions/approaches and their contributions to recovery;
  • Assesses individual needs, creates PSR and recovery oriented plans and measures outcomes;
  • Works with individuals to maximize access to work leisure, education, and community;
  • Fosters recovery, wellbeing, and equity within systems and across the social; determinants of health;

Job Types:
Full-time, Permanent


Pay:
$30.20-$31.89 per hour

Expected hours: 35 per week
