OR graduate of recognized Social Service Work diploma program,
OR Bachelor's Degree in Social Work, Gerontology.

Role Responsible for_

  • Liaison with community agencies and partners regarding the placement and ongoing support of prospective residents
  • Providing information to potential applicants/families regarding placement process including conducting tours of the Home.
  • Facilitating and completing the admission/discharge process for all permanent and shortterm admissions in collaboration with the Director of Nursing (DoN), Director of Clinical (DoC) Services and the Charge Nurses
  • Liaise with Resident/PGT/POA to assist in accessing resident banking information on or offsite
- trust account and monitor funds.

  • Assists Director of Care with investigations and reporting about financial abuse
  • Assist in resident personal supply purchases
- on and off-site.

  • Triage family and resident's concerns and complaints and support DoN and DoC in addressing concerns about various areas of care, and physical environment
  • Scheduling resident capacity assessments/evaluations.
  • Policy reviews as requested by DoN or Administrator for policies relevant to activities associated with resident wellbeing.
  • Conducting regular audits to ensure compliance with the FLTCH Act and Regulations.
  • Identify and organize visits from external supports such as dental clinics, optometrists, counselling, etc.
  • Manage bookings of locations within the home for families/POAs/Residents (Guest Room, Dining Room, Hairdressing appointments)
  • Create, conduct and analyze annual family satisfaction surveys

Participates as a member of the inter-professional care team in_
- developing and implementing resident care plans,
- determine the need for resident capacity assessments/evaluations
- resident mediation and investigation in a person-centric approach
- providing mediation in difficult resident/family situations
- investigation of alleged resident-to-resident abuse

  • Provide support in the deescalation and management of emergent situations such as Code white or other situations.
  • Facilitates internal resident transfers in collaboration with the resident/substitute decision maker, facility staff and other residents
  • Attending Resident Care Meetings, Family Council Meetings
  • Works in compliance with the Health & Safety Act and its regulations in safely performing duties and follows all health and safety policies, procedures and legislation including, wearing the appropriate Personal Protective Equipment (PPE), as required
  • Works in compliance with the Fixing Long-Term Act and Regulations 246/22

Responsible for providing individual and or group support for_
- residents and family members and shares findings with care teams concerning the unique transition support required by the resident to foster adjustment to the new environment
- residents experiencing psycho-social issues (depression, anxiety, loneliness, bereavement) within the scope as set out by the College of Social Workers and Social Service Workers

Ongoing Education, Expertise & Support_

  • Provides expertise, education and support to the interprofessional Care team as it relates to the Code of Ethics and Standards of Practice Handbook as set out by the College of Social Workers and Social Service Workers
  • Follows the Code of Ethics and Standards of Practice Handbook as set out by the College of Social Workers and Social Service Workers
  • Participates in ongoing learning and skills development to fulfill the requirements of the regulatory body
  • Provides supervision to Social Service Worker/Social Worker students, as assigned
