SLCS is currently looking for a Full Time Team Leader in the SIL program on a 1-year contract as well as a permanent position.
As a leader of this interdisciplinary team you will support the direct service employees in the delivery of support services and crisis intervention to adults with a developmental disability and/or mental health struggles.
Your responsibilities will include supporting the team with developing care plans that assess clients' skills and needs particularly with regard to their mental health, developmental delay, fetal alcohol spectrum, acquired brain injury and addiction issues.
You will liaise with other team members to coordinate care for clients.In addition to the leadership elements of the position, you will be responsible to ensure the safety and security of the clients, other staff and the community during your work days.
Reporting to the Program Manager, you will provide front line leadership, regular coaching and direction to direct service employees from the Supported Independent Living program, focused on the day-to-day delivery of case management, problem-solving and decision making, crisis intervention, and program planning/implementation.
You will work closely with all team members and other team leaders of the SIL program to ensure productivity, quality client care, as well as training and orientation to new employees.
You will provide individual and team coaching, assisting with coordination of daily client support assignments, provide guidance on the written documentation of SMART goals to Case Workers, be involved in team development activities, and model a positive workplace culture.
In addition to assisting with the coordination of daily client supports, you will help to manage the flow of communication across teams and programs.
The Team Lead position will also provide shared responsibility to ensure adequate staffing coverage for immediate needs (i.e. sick calls), and work collaboratively and effectively with the Program Manager to ensure the provision of quality services and program delivery. This position will require you to be in the community at times, and you will be expected to travel between SLCS facilities and to community locations.
Required Qualifications
- University degree or 3year College diploma from an accredited institution
- Standard First Aid/CPR
- Level C
- Valid G Class Driver's License and a reliable vehicleC
