    We are currently looking to fill a Casual opportunity for a Social Worker at Fellburn Care Centre located in Burnaby, BC .

    Reporting to the Manager, Clinical Operations, the Social Worker assists residents and their families to adjust to the stress associated with chronic illness/disability, relocation, and institutional life.


    Completes and updates psycho-social assessments of residents and clients; make referrals to appropriate community agencies, as required.
    # Attends interdisciplinary care conferences and consults with others, both internally and externally in relation to resident and client services.
    # Assists with the development, implementation and review of resident and client care plans. Records actions taken on appropriate resident and client health records.
    # Participates in resident and client discharge planning.
    # Attend meetings and participate in hospital and community planning, as assigned. Assists in research projects and education programs, as assigned.
    # Assists in the development and provision of relevant education programs for residents/clients, families and staff Social Workers.
    # Education, Training and Experience

    Bachelor's Degree in Social Work from an accredited school of Social Work.

    Two year's recent, related experience working as a Social Worker in a long term care setting inclusive of clinical training and experience in individual, marital/family and group modalities of social work practices.

    Current full registration with the BC College of Social Workers.
    Ability to communicate effectively both verbally and in writing.
