About the Role:


Reporting to the Assistant Client Services Manager, the Client Care Supervisor (CCS) is responsible for the overall management and functioning of Home Support Services, including but not limited to: monitoring, training and assisting Personal Support Workers (PSWs) in the delivery of Personal Support and Homemaking Services to Clients.


Who We are Looking For:


  • An RN or RPN with a current College of Nurses Ontario (CNO) registration in good standing OR;
  • A Social Worker with a current Ontario College of Social Workers registration in good standing
  • Current cardiopulmonary resuscitation certification, a vulnerable sector police check and a Tuberculosis test with negative results
  • Recent experience in a community health care setting is preferred
  • Case management skills
  • Valid driver's license and reliable transportation
  • Daily traveling in assigned area is required

A Day-to-Day Look at the Role:

  • Supervision and mentoring of home support Health Care Professionals
  • Communicates with client, family, case managers and staff regarding scheduling, care plan goals and service delivery, as needed
  • Resolves problems and incidents with stakeholders in timely manner
  • Prepares interim reports on clients as required
  • Ensures that all correspondence is accurate
  • Accurately dispatches client information to Service Provider Personnel
  • Provides counseling and advocacy to clients and families
  • Monitors and evaluates client care by visiting clients on a regular basis
  • Provides input for annual performance appraisals
  • Complete three (3) month probationary performance appraisals of Personal Support Workers
  • Identifies work performance problems and takes corrective action
  • Represents RNS Health Care Services (RNS) at community events, meetings and case conferences
  • Promotes RNS through the use of promotional materials and by liaising with the public and health care practitioners in a professional manner
  • Identifies educational needs of Personal Support Workers and advises Home Support Manager and Training & Development Coordinator of additional training that needs to be added to orientations and/or the education plan
  • Collaborates with all members of the multidisciplinary team to ensure optimum client care
  • Recommends expenditure of funds and/or time for Personal Support Workers in order that they may participate in educational opportunities available to them
  • Participates in the Quality Improvement Program
  • Participates on committees and undertakes projects and assignments as determined by Management
  • Maintains Professional Standards as directed by their Professional College, Regulated Health Professions Act, 1994 and applicable legislation
  • Participate in clinical oncall rotation as laid out in Policy HR-ER31 On-Call Policy

Excellent oral, written, communication and computer skills

  • Demonstrated problem solving and decision making skills
  • Ability to plan, prioritize and organize
  • Flexibility in work assignment
  • Strong leadership skills
  • Knowledge of Home and Community Care Support Services, formerly LHIN is considered an asset
  • Experience in adult education is considered an asset

$55,000.00-$68,000.00 per year
