    Toronto General Hospital (TGH) is leading the way in cardiac care, organ transplants and the treatment of complex patient needs. Home to three of UHN's major program groupings (Heart and Circulation, Transplantation and Advanced Medicine and Surgery), our highly skilled teams are committed to UHN's vision of achieving global impact. The glomerulonephritis clinic is a provincial resource designed to improve the healthcare of patients with these rare and complex kidney conditions. The nursing clinical coordinator in the glomerulonephritis clinic, in collaboration with the healthcare team, will facilitate patient care, improve the education of the patient and referring physicians in these kidney conditions with the ultimate goal of preventing kidney disease progression. As a member of the Ambulatory Clinic (GN) team, the responsibilities of the CLINICAL COORDINATOR, include: utilizing and applying current specialized knowledge and expertise on recent advances in nursing practice to facilitate the effective management of clinical activities, the coordination of patient care across the continuum and continuity in patient care management; performing patient care responsibilities; performing teaching/educational responsibilities; participating actively in Quality Improvement initiatives; participating in academic and research activities/projects; performing administrative responsibilities; performing cross-functional and/or other duties consistent with the job classification, as assigned or requested.

    Qualifications

  • Bachelors of Science Degree in Nursing
  • Completion of a recognized Masters degree nursing program preferred
  • Certificate of competency (licensure) from the College of Nurses of Ontario
  • Completion of Basic Cardiac Life Support ( program, Level II Rescuer certification
  • Up to three (3) years of relevant experience including experience working in an advanced Clinical practice role
  • Three (3) to Five (5) years experience in nephrology nursing required
  • Effective organizational, interpersonal and communication skills
  • Excellent decision-making and problem solving skills
  • Excellent time management skills
  • Excellent teaching skills with the ability to teach one-on-one and/or groups
  • Ability to function collaboratively within a multi-disciplinary team and/or independently
  • Ability to work in a self directed manner
  • Demonstrated awareness of and adherence to professional and ethical standards
  • Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand
  • Computer literacy
  • Research experience preferred
