About Program:

Rapid Response Saving Lives (RRSL) is a pilot project. This program offers pre-emptive mental wellness support and post-hospital discharge support to individuals and their family members. The program is based on the principles of hope, recovery, empathy, self-determination, and respect.

Case managers and Peer Support Workers will work with the clients experiencing substance abuse and mental health issues and ready to work toward their recovery.

RRSL staff will provide linguistically and culturally competent mental wellness psycho-educational support and post-hospital discharge support to patients and their families to achieve their recovery and rehabilitation goals.


About Position:


This position will provide rapid response with pre-emptive wellness support and post-hospital discharge support mainly focused on addiction and mental health case management.

This position aims to provide services to clients who are most vulnerable and repeat emergency department users.

The Case Manager will work with the Hospitals regarding post-discharge support, support clients and their significant others, provide wrap-around support, and link to other services.

The case Manager will actively engage the peer support worker to provide prompt and appropriate support to vulnerable patients post-hospital discharge.

The case manager and peer support worker will work collaboratively to facilitate the clients' recovery and rehabilitation process.

Case Manager will provide client-centered short-term case management in a timely, high-quality, holistic, and integrated manner that will promote clients' well-being and reduce repeat visits to hospital emergency departments.


Qualifications:


  • University Degree or College diploma in social services or health services field is required
  • Preferred Registered Health professionals and adhere to standards of practice as per
- designated college

  • Preferred 2 years of experience in the related field, or a combined equivalent of education
- and experience

  • Demonstrate understanding of the concepts of a clientcentered approach and create safe
  • Demonstrate knowledge about addiction treatment services and available community
- resources

  • Proven knowledge and skills in assessment, care plan development, case management
- and supportive counselling

  • Familiarity with Ontario Common Needs Assessment (OCAN), Drug Addiction Treatment
  • Information System (DATIS), GAIN Q3-MI is an asset.
  • Knowledge of South Asian languages including Punjabi, Hindi and Urdu will be an asset
  • Consistent commitment to professional development
  • Strong interpersonal skills, be able to work independently and within a team
  • Strong verbal and written communication skills including presentation skills and the ability
- to communicate information to a variety of stakeholders effectively

  • Excellent computer skills with advanced proficiency in Microsoft Office
  • Familiarity with Cloud technology and SharePoint will be an asset

Requirements:


  • Must possess a valid Ontario Driver's License, have regular access to a reliable vehicle and provide proof of adequate vehicle insurance
  • A satisfactory current Vulnerable Sector Screening (Criminal Record Check) report
  • Current CPR and First-Aid certification
  • Vaccines (COVID19 and others) are encouraged.

  • Support clients' post hospital discharge through the provision of shortterm case management
  • Psychoeducational group support for clients and their families
  • Actively engage the peer support worker with clients to facilitate their recovery and rehabilitation process

  • Support clients in reaching their goals through interventions such as advocacy, referral to appropriate community resources/programs, supportive counselling, family support and psychoeducation
