Health PEI is looking for skilled Physiotherapists to join our collaborative teams across the provinceAcute Care:
PTs who choose to work in a hospital setting will perform assessments, create and progress treatment plans and are strongly involved in care coordination and discharge planning
Services include:
orthopedics, pre/post-surgical care, rehabilitation (including conditions like spinal cord injuries, amputations, etc), ICU support, pediatrics, and more.
Patient Medical Homes:
Seventeen (17) Island primary care sites have adopted Patient Medical Homes, initiating a shift from solo family practices to collaborative care, enhancing Islanders' access to healthcare.
Physiotherapists within the PMH model will collaborate with dietitians, social workers, nurses, and other professionals. The PT role in PMHs is different from the traditional model that patients may have experienced in the past.PTs assume accountability for the initial assessment and management of a pre-defined patient group the physician/nurse practitioner would typically have assessed.
Identification of patients appropriate for direct access to the PT is defined in collaboration with the primary care provider.including transition and discharge planning.
Participating in quality improvement; recognizing the need for involvement of other health care professionals and making referrals/consults to bring them into circle of care.
Conducting home visits and site visits.Supervising Rehabilitation Assistants.
Training, orientating, and providing preceptorship/mentorship/teaching for various students, staff, colleagues, other health professionals.
Must have successful completion of a Master's Degree or equivalent in Physiotherapy.
Must be eligible for licensing with the Prince Edward Island College of Physiotherapists.
Demonstrated commitment to continuous professional development will be considered an asset.
Ability to develop and lead training sessions for clients and staff.
