Description

Responsibilities:


  • Provide support and guidance to clients.
  • Conduct assessments of clients' needs and develop individualized service plans.
  • Maintain accurate records of client information and progress.
  • Conduct home visits and provide inhome support services.
  • Collaborate with other agencies to coordinate services for clients.
  • Provide crisis intervention and advocacy services.
  • Assist clients in accessing community resources.
  • Develop and maintain relationships with clients.

Job Types:
Full-time, Part-time, Permanent, Fixed term contract

Contract length: 6 months

Part-time hours: 40 per week


Salary:
$18.00-$20.00 per hour

  • 8 hour shift
