Eviction Prevention Case Manager Contract full time position (35 hours per week)


Contract Start date:
Immediately;
Contract End date: March 31, 2024 (possibility of extension, full benefits after probation)

Supervisor, Eviction Prevention in the Community (EPIC)


Background
ACSA is a nonprofit, multi service agency addressing needs and empowering children, youth, newcomers, homeless, and otherwise underserved communities.

Our core programs include housing and outreach programs, food security programs, newcomer services, youth outreach and therapy programs, child and family programs, early years' programs, computer access programs, and community engagement.


Summary of Job Functions
The Eviction Prevention Case Manager will provide intensive intervention services for people who are at imminent risk of eviction.

The case manager will support newcomers, families and individuals that are in a process of eviction and who have been referred by the City of Toronto, to retain their tenancy, or relocate and help mitigate the effects of an unsustainable tenancy.

The case manager will work within a housing-first, client-centred, and harm reduction approach within a team.

The case manager will assist the tenant to create plans that will help prevent future eviction and homelessness through housing assessment, stabilization, goal planning, and linkages to community services.


Duties include:


  • Receive and follow up on referrals according to City of Toronto EPIC (Eviction Prevention in the Community) processes; follow all Project processes and requirements;
  • Manage a caseload of up to 20 clients for rapid response eviction prevention and housing stabilization, providing appropriate supports and referrals to services
  • Provide individualized and flexible case management services, through a housingfirst approach
  • Assess eviction status, analyze issues and develop case plans for eviction prevention
  • Assess clients' immediate and ongoing needs for supports and provide managed referrals for: follow up case management services, voluntary trusteeship
  • Advise clients on program requirements and rights and responsibilities of tenant and landlord
  • Accompany clients to Landlord and Tenant Board hearings to provide support and advocacy as needed
  • Advocate, mediate and negotiate with landlords and housing providers with payment plans and for sustaining tenancies
  • Provide rehousing supports if eviction cannot be prevented
  • Maintain accurate case management and program records, using SMIS
  • Meet reporting timelines and requirements of funding bodies and the organization
  • Prepare statistical and qualitative reporting as required by funder and the organization
  • Prepare monthly statistical reports as required by funding bodies and the organization
  • Attend joint meetings and trainings as set out by the City of Toronto
  • Work collaboratively with colleagues and participate in team meetings
  • Attend Case Manager/ Supervisor meetings
  • Perform the responsibilities of the position within the legislative and regulatory standards set out in the applicable Acts and consistent with the policies of ACSA
  • Protect own health and health of others by adopting safe work practices, reporting unsafe conditions immediately. Follow all guidelines for employees and employers as legislated under the Ontario Occupational Health and Safety Act.
  • Other duties as required.

QUALIFICATIONS

  • Post-Secondary degree in Social Services or related discipline and/or the equivalent combination of education and experience
  • Strong understanding of market rent and subsidized housing systems and policies, keeping uptodate on emerging trends and changes; knowledge of the RTA
  • Strong analytical and problemsolving skills
  • Excellent oral and written communications skills
  • Experience with maintaining statistics and writing program reports
  • Demonstrated understanding and commitment to harm reduction framework and housing first approach.
  • Demonstrated knowledge and experience of eviction prevention, housing access, housing stabilization and case management for housing retention
  • Experience providing housing support services to families, newcomers and people who are or have been homeless or at risk of homelessness
  • Experience working with vulnerable and diverse communities that are dealing with issues related to racialization, poverty, violence, mental health, social isolation and exclusion
  • Tact, diplomacy and assertiveness in working with clients under stress
  • Ability to manage issues tactfully, and relationships with professionalism, and resolve conflicts
  • Competency in Microsoft Office, including Word, Outlook, Excel and PowerPoint as well as data reporting programs
  • Ability to work independently and as part of a close team
  • Valid G driver's license with clear driver's abstract and a vehicle for daily use for client meetings and appointments
  • Valid Police Records Check submitted wit
