The Salvation Army Toronto Grace Health Centre (TGHC) is a mission of The Salvation Army that provides specialized and integrated care and services to individuals with multifaceted chronic or persistent illnesses that require Complex Continuing Care, Post Acute Care Rehabilitation, Palliative care and Transitional care services.

***The Toronto Grace Health Centre is currently seeking a Social Worker who is a member of the interprofessional team to provide compassionate, safe, quality, patient centered care to individuals living with complex mental health and chronic illnesses and who require special case management to help support their integration in the community.


MAIN RESPONSIBILTIES

  • Provide a range of social work services including psychosocial assessment, crisis intervention, discharge planning and counseling to patients, residents and their families.
  • Act as liaison and assist patient, residents and staff in accessing community resources and agencies.
  • Provide comprehensive discharge planning services for patients, residents and families in collaboration with the interprofessional team as well as internal and external stakeholders as applicable
  • Provide indepth transfer of accountability and progress reports to ensure a seamless transition of patient and family care.

QUALIFICATIONS AND EDUCATION:


  • Master's degree in Social Work from an accredited School of Social Work, or equivalent degree as recognized by the Ontario College of Social Worker is required.
  • Flexible schedule is required.
  • Experience in a hospital or health care setting.
  • Experience with case management and discharge planning for people living with mental health illness
  • Knowledge of the acute, community, and mental health and addictions management sectors as well as understanding of community resources is required
  • Demonstrate ability to welcome change and continuous improvement.
  • Experience in a mental health setting is required.
  • Strong communicator with proven problem solving abilities who works efficiently in an interprofessional setting.
  • Member in good standing with the OCSWSSW.
  • Knowledgeable of cultural system perspective that includes family, community, religious and spiritual resources.
  • Excellent interpersonal skills as demonstrated by your ability to develop and maintain positive relationships with patients, residents /families, and members of the health care team.
  • Exceptional time management skills.
  • Excellent communication skills (oral and written) facilitating effective communication in case reporting and assessments, with patients, residents, families, other healthcare team members and volunteers.
  • Selfmotivated, team player, and ability to thrive in a fastpaced environment.
  • High level of integrity, leads by example and exemplifies our Core Organizational Values.

Salary:
$37.00-$45.00 per hour
