OVERVIEW:

Reporting to the South Regional Care Manager, the Alternative Care Case Manager will be responsible for providing intensive case support, quality assurance measures, service direction, and support for Caregivers and children under the South Regional Care Program.

In addition, the Alternative Care Case Manager will develop and facilitate pre-service training and orientation for new Caregivers in the South Regional Care Programs.


DUTIES AND RESONSIBILITIES

Case Manager Duties:


  • Conduct monthly case planning meetings in the foster homes with the assigned Southeast Child and Family (SECFS) workers and other collaterals.
  • Licensing of Homes;
  • Complete Annual reviews for each home
  • Complete addendums for each home, as needed.
  • Complete Foster Care Licensing Closures, as needed.
  • Complete Home studies, as needed
  • Complete background checks for the caregivers
  • Maintain an accurate understanding of the services provided in each home and effectively communicate to the South Regional Care Manager and SECFS.
  • Collaborate with SECFS workers in delivering care plans for each child and/or family, while remaining focused on child's best interests.
  • Monitor and follow up with ongoing needs in the homes.
  • Collect and review monthly reports and incident reports for each home.
  • Share the incident reports and monthly reports with the assigned SECFS workers.
  • Complete detailed case notes for each home and provide for monthly review to South Regional Care Manager.
  • Review program memos and directives with House Parents.
  • Attend meetings with children and House Parents as necessary (lawyer, court, doctor appointments, assessments)
  • Ensure all Caregiver documentation requirements, including reporting, are completed in a timely and accurate manner.
  • Ensure outcomes, assessments and service plans are current and up to date.
  • Other duties as assigned.
  • Regular travel is required for this position (Winnipeg and surrounding areas).

Resource Duties:


  • Assist in planning and facilitating monthly Caregiver meetings.
  • Assist with Caregiver training and programming.
  • Conduct home study assessments, according to guidelines provided by the organization and placing agency.
  • Identify and make recommendations for additional training sessions for care providers and support staff in the homes.
  • Maintain effective communication with program staff and Caregivers as required to maintain awareness of program needs.

QUALIFICATIONS:


  • BSW degree or equivalent (similar education and experience working in the field will be considered)
  • Knowledge in the areas of colonization and intergenerational trauma
  • Excellent conflict resolution skills
  • Knowledge of the Child and Family Services (CFS) sector and legislation
  • Good understanding of foster care standards and related licensing paperwork
  • Demonstrate knowledge and/or understanding of Indigenous perspectives, First Nations culture and traditional philosophies.
  • Experience developing and supporting a strong, respectful teambased approach to service delivery.
  • Experience working with families and navigating complex situations.
  • Strong proficiency in Microsoft Office, word processing (MS Word), Spreadsheets (Excel), Email (Outlook) and Internet.
  • Excellent communication, organization, problem solving and team skills.
  • Ability to speak Anishinaabe is an asset.
  • Successful applicant(s) will also be required to provide proof of COVID19 vaccination prior to start date.
  • Must a valid Manitoba driver's license and a reliable vehicle.
  • Must have a current satisfactory Child Abuse Registry Check, Adult Abuse Registry Check, Criminal Record check (including Vulnerable Sector Search), and Prior Contact Check as conditions of employment. (This does not necessarily mean you will not be eligible to work for SAI).
